摘 要:目的 研究基于超声造影定量分析评估颈动脉低回声斑块与脑梗死复发的相关性。方法 选取116例首次确诊脑梗死患者,均行超声造影定量分析。随访1年,根据脑梗死是否复发分为复发组(n=50)和未复发组(n=66),结合两组患者的一般临床资料、超声造影时间-强度曲线相关参数,并采用受试者工作特征(ROC)曲线分析超声造影时间-强度曲线相关参数对脑梗死复发的诊断效能。结果 (1)两组患者性别、美国国立卫生研究院卒中量表(NIHSS)评分、小卒中、颈动脉斑块部位、颈动脉斑块位置、颈动脉斑块新生血管位置、颈动脉斑块厚度、颈动脉斑块长度、高血脂史、甘油三酯水平、胆固醇水平、糖尿病病史、空腹血糖、冠心病病史、吸烟史和饮酒史等差异无统计学意义(P>0.05),复发组年龄、高血压史、平均动脉压均明显高于未复发组,差异具有统计学意义(P<0.05);(2)复发组强度峰值、增强强度(EI)、斜率及强度比值(Ratio)均明显高于未复发组,差异具有统计学意义(P<0.05);(3)经多因素Logistic回归分析发现,高血压、EI及Ratio是脑梗死复发的独立危险因素;(4)EI、Ratio及EI与Ratio联合检测均可用于预测脑梗死复发,与EI、Ratio比,EI与Ratio联合检测预测脑梗死复发的曲线下面积(AUC)、灵敏度和特异度均较高。结论 超声造影定量分析可用于预测脑梗死复发,EI与Ratio联合检测预测脑梗死复发的效能较高。Objective To evaluate the correlation between carotid hypoechoic plaques and cerebral infarction recurrence based on quantitative analysis of contrast-enhanced ultrasound(CEUS).Methods 116 patients with cerebral infarction diagnosed for the first time in Shaanxi Provincial People's Hospital were selected as the study subjects,all patients underwent contrast-enhanced ultrasound quantitative analysis.The patients were followed up for 1 year,according to whether the cerebral infarction recurred,they were divided into the recurrence group(n=50) and non recurrence group(n=66).The general clinical data and relevant parameters of CEUS time intensity curve of 2 groups were compared,and the diagnostic efficacy of relevant parameters of CEUS time intensity curve for cerebral infarction recurrence was analyzed by receiver operating characteristic(ROC) curve.Results(1)There was no significant difference in gender,National Institutes of Health Stroke Scale(NIHSS),stroke,carotid plaque location,carotid plaque neovascularization location,carotid plaque thickness,carotid plaque length,hyperlipidemia history,triglyceride,cholesterol,diabetes history,fasting blood glucose,coronary heart disease history between 2 groups(P>0.05).The proportion of hypertension history and average arterial pressure were significantly higher than those in the non recurrent group,and the difference was statistically significant(P<0.05).(2)The peak intensity,enhanced intensity(EI),slope,and intensity ratio of the recurrent group were significantly higher than those of the non recurrent group,and the differences were statistically significant(P<0.05).(3)The multiple Logistic regression analysis found that hypertension,EI and Ratio were independent risk factors for cerebral infarction recurrence.(4)EI,Ratio and the combined detection of EI and Ratio could be used to predict the recurrence of cerebral infarction.Compared with EI and Ratio,the area under the curve(AUC),sensitivity,and specificity for predicting the recurrence of cerebral infarction in the co
