摘 要:目的 研究旨在探讨帕金森病(PD)经颅超声检查中黑质强回声与PD临床特征之间的相关性,以进一步了解黑质强回声在PD诊断和病情评估中的意义,为PD早期诊治提供新的思路。方法 研究纳入确诊的50例原发性PD患者作为实验组,并选定性别及年龄比例与实验组相匹配的50例健康成年人作为对照组。对其临床特征进行详细记录,包括年龄、性别、病程、症状表现等。同时,采用经颅超声技术对所有患者进行黑质强回声检测。采用相关性分析、卡方检验和线性回归分析等方法,分析黑质强回声与PD临床特征之间的关联性。结果 在黑质强回声检测方面,发现79例患者存在黑质强回声现象。在临床特征方面,存在黑质强回声的患者其病程更长、症状表现更严重(P<0.05)。相关性分析显示,黑质强回声与PD病程之间存在显著正相关性(r=0.672,P<0.01)。结论 黑质强回声与PD临床特征之间存在显著关联性。因此,黑质强回声检测可以作为评估PD病情严重程度和病程进展的重要指标。建议在未来的临床实践中进一步推广应用黑质强回声检测技术,以提高PD诊断和治疗的灵敏度。Objective To investigate the correlation between substantia nigra hyperechogenicity and clinical features of Parkinson's disease(PD) in transcranial ultrasound,so as to further understand the significance of substantia nigra hyperechogenicity in diagnosis and evaluation of PD,and provide new ideas for early diagnosis and treatment of PD.Methods Fifty patients with PD diagnosed were selected as the experimental group,and 50 healthy adults matching the gender and age ratio of the experimental group were selected as the control group.The clinical characteristics of the patients were recorded in detail,including age,gender,course of disease,symptoms and so on.At the same time,transcranial ultrasound was used to detect the substantia nigra in all patients.Correlation analysis,Chi-square test and linear regression analysis were used to analyze the correlation between substantia nigra hyperechogenicity and clinical features of PD.Results In the detection of substantia nigra hyperechogenicity,79 patients were found to have substantia nigra hyperechogenicity.In terms of clinical features,patients with substantia nigra hyperechogenicity had a longer course of disease and more severe symptoms(P<0.05).Correlation analysis showed that there was a significant positive correlation between substantia nigra hyperechogenicity and the course of PD(r=0.672,P<0.01).Conclusions There is a significant correlation between substantia nigra hyperechogenicity and clinical features of PD.Therefore,detection of substantia nigra hyperechogenicity can be used as an important indicator to evaluate the severity and course of PD.In order to improve the accuracy and effectiveness of diagnosis and treatment of PD,it is suggested that the technique of substantia nigra hyperechogenicity detection should be further popularized in future clinical practice.
