疏肝健脾汤联合异甘草酸镁对原发性胆汁性肝硬化免疫系统的影响

摘　　要：目的探讨疏肝健脾汤联合异甘草酸镁对原发性胆汁性肝硬化患者免疫系统的影响。方法将甘肃省中医院2019年1月一2022年1月收治的121例原发性胆汁性肝硬化患者随机分为试验组60例和对照组61例,两组均给予基础治疗,对照组加用异甘草酸镁,试验组在对照组基础上给予疏肝健脾汤治疗。于治疗前、治疗半年检测两组血清免疫球蛋白(Ig)G、IgA、IgM水平和补体C3、C4水平,评估两组治疗前后中医证候积分及疗效,统计治疗期间不良反应发生情况。结果治疗后,两组IgG、IgA、IgM水平均明显低于治疗前(P<0.05),试验组均明显低于对照组(P<0.05);两组C3、C4水平均明显高于治疗前(P<0.05),试验组均明显高于对照组(P<0.05)两组中医证候积分均明显低于治疗前(P<0.05),试验组明显低于对照组(P<0.05);试验组中医证候疗效明显高于对照组(P<0.05);试验组不良反应发生率明显低于对照组(P<0.05)。结论疏肝健脾汤联合异甘草酸镁治疗可有效降低原发性胆汁性肝硬化患者的血清免疫球蛋白水平,升高血清补体水平,改善中医证候积分,提高中医证候疗效,且可减少不良反应,安全性和有效性均较高。Objective To investigate the effect of thin Shugan Jianpi Decoction(疏肝健脾汤)combined with magnesium iso-glycyrrhizin on the immune system in patients with primary biliary cirrhosis.Methods 121 patients with primary biliary cirrhosis admitted Gansu Provincial Hospital of Traditional Chinese Medicine from January 2019 to January 2022,Randomized into study group(60 cases)and control group(61 cases),In both groups,The control group added magnesium isoglycyrrhizin to this basis,The study group was treated on the basis of the control group,The serum immunoglobulin(Igg)G,IgA,IgM and complement C3 and C4 levels were tested before and after half a year of treatment,To evaluate the TCM syndrome score and efficacy of both groups before and after treatment,The occurrence of adverse reactions during the treatment period was counted.Results The levels of IgG,IgA and IgM were significantly lower than those before the treatment(P<0.05),Moreover,the study group was signifi-cantly lower than the control group(P<0.05),C3 and C4 levels were significantly higher than before treatment(P<0.05),and the study group was significantly higher than the control group(P<0.05),TCM syndrome score after both groups were sig-nificantly lower than before treatment(P<0.05),and the study group was significantly lower than the control group(P<0.05),the efficacy of TCM syndrome in the study group was significantly higher than that in the control group(P<0.05),the incidence of adverse reactions in the study group was significantly lower than that in the control group(P<0.05).Conclusion The treatment of Shugan Jianpi Decoction combined with magnesium isoglycyrin can effectively reduce the serum im-munoglobulin level of primary biliary cirrhosis patients,increase the serum complement level,improve the syndrome score of TCM syndrome,and reduce the occurrence of adverse reactions,with high safety and effectiveness.
