广西地区慢性病空巢老人居家健康照护需求及影响因素分析

摘　　要：目的调查慢性病空巢老人的健康照护需求现状并分析其影响因素,为慢性病控制政策的制定提供循证依据。方法采用便利抽样的方法选取广西地区居家老年人1050例,应用西部护理联盟老年人健康照护需求研究调查组设计的老年人健康照护需求调查问卷进行调查,对其中的338例慢性病空巢老人的调查数据进行分析,了解其经济、医保情况、健康状况、自理能力、护理需求及养老意愿,并对慢性病空巢老人居家健康照护需求可能的影响因素进行分析。结果慢性病空巢老人护理需求排前3位的分别为安全需求、健康教育需求、尊重及自我发展;需求最低的是康复训练与指导需求。Logistic多因素回归提示:年龄、月收入、锻炼、跌倒史是影响社区老年人护理服务需求高低的独立影响因素。结论目前空巢老人居家健康照护需求呈现多元化特征,满足其多方位的健康照护需求应充分考虑到老年人的年龄、月收入等主要影响因素。Objective To investigate the current situation of health care needs among empty nest elderly with chronic diseases and its influencing factors,in order to provide evidence-based basis for the formulation of chronic disease control policies.Methods Convenience sampling was used to select 1050 empty nest elderly people living at home in Guangxi region.A survey questionnaire on elderly health care needs designed by the Western Nursing Alliance Elderly Health Care Needs Research Survey Group was used.The survey data of 338 empty nest elderly people with chronic diseases who lived at home were analyzed to understand their economic,medical insurance,health status,self-care ability,nursing needs,and willingness to care for the elderly.Influencing factors for home-based health care needs in them were analyzed.Results The top three nursing needs for empty nest elderly people with chronic diseases who lived at home were safety needs,health education needs,and respect and self-development needs.The lowest nursing need was rehabilitation training and guidance.Logistic multiple regression suggested that age,monthly income,exercise,and history of falls were independent influencing factors for the nursing care demand among empty nest elderly people in the community.Conclusion Currently,the home health care needs of empty nest elderly people exhibit diversified characteristics.To meet their multi-dimensional health care needs,the main influencing factors like age and monthly income of the elderly should be fully considered.
