摘 要:目的:探讨轮状病毒阳性患儿发生轻度胃肠炎伴婴幼儿良性惊厥(BICE)的影响因素。方法:选取2017年1月—2023年6月广西壮族自治区人民医院收治的18例轮状病毒抗原阳性BICE患儿作为BICE组,选取同期收治的85例轮状病毒性肠炎(MRE)患儿作为MRE组。收集并比较两组基本资料及实验室检查结果,采用多因素Logistic回归分析轮状病毒阳性患儿发生BICE的独立影响因素,采用受试者操作特征曲线分析独立影响因素诊断轮状病毒阳性BICE的效能。结果:BICE组呕吐次数多于MRE组,Na^(+)、Cl^(-)水平低于MRE组,尿酸(UA)水平高于MRE组,差异有统计学意义(P<0.05)。Cl^(-)水平降低、UA水平升高是轮状病毒阳性患儿发生BICE的独立影响因素(P<0.05),Cl^(-)、UA对轮状病毒阳性BICE有一定诊断价值,二者联合检测对轮状病毒阳性BICE有较高诊断价值。结论:低Cl^(-)、高UA是轮状病毒阳性患儿发生BICE的独立影响因素,可用于轮状病毒阳性BICE的辅助诊断,且联合检测诊断效能较高。Objective:To explore the influencing factors of benign infantile convulsions associated with mild gastroenteritis(BICE)in rotavirus-positive children.Methods:A total of 18 children with rotavirus antigen-positive BICE admitted to the People's Hospital of Guangxi Zhuang Autonomous Region from January 2017 to June 2023 was selected as the BICE group,and 85 children with mild rotavirus enteritis(MRE)during the same period were selected as the MRE group.The basic data and laboratory test results of the two groups were collected and compared.Multivariate Logistic regression analysis was used to analyze the independent influencing factors of BICE in rotavirus-positive children.The receiver operating characteristic curve was used to analyze the efficacy of independent influencing factors in diagnosing rotavirus-positive BICE.Results:The number of vomiting in the BICE group was higher than that in the MRE group,the levels of Na^(+)and Cl^(-)in the BICE group were lower than those in the MRE group,the levels of uric acid(UA)in the BICE group were higher than those in the MRE group,and the difference was statistically significant(P<0.05).The decreased Cl^(-)level and the increased UA level were independent influencing factors for the development of BICE in rotavirus-positive children(P<0.05).The Cl^(-)and UA had certain diagnostic value for rotavirus-positive BICE,and the combined detection had a high diagnostic value for rotavirus-positive BICE.Conclusion:Low Cl^(-)and high UA are independent influencing factors for BICE in rotavirus-positive children,and can be used as an auxiliary diagnosis for rotaviruspositive BICE,and the combined detection has a high diagnostic efficacy.
