复方黄柏液坐浴辅助内口封闭术对高位肛周脓肿患者术后疼痛、创面愈合及肛门括约肌功能的影响

摘　　要：目的探讨高位肛周脓肿患者采用复方黄柏液坐浴辅助内口封闭术治疗对术后疼痛、创面愈合及肛门括约肌功能的影响。方法选择2020年6月至2023年6月扬州市第二人民医院收治的150例高位肛周脓肿患者作为研究对象,按照随机数字表法将其分为对照组与观察组,每组各75例。对照组行内口封闭术+高锰酸钾坐浴治疗,观察组行内口封闭术+复方黄柏液坐浴治疗。比较两组创面愈合情况、术后疼痛、肛门括约肌功能、炎症因子[白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]及血清指标[表皮细胞生长因子(EGF)、血清转化生长因子-β(TGF-β)、血管内皮细胞生长因子(VEGF)]。结果观察组的创面水肿、渗液评分均低于对照组,愈合时间短于对照组,术后疼痛、肛门括约肌功能评分均低于对照组,差异有统计学意义(P<0.05)。术前两组患者的IL-6、TNF-α、CRP、EGF、TGF-β、VEGF水平比较,差异无统计学意义(P>0.05);术后两组患者的IL-6、TNF-α、CRP均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后两组患者的EGF、TGF-β、VEGF水平均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论高位肛周脓肿患者采用内口封闭术+复方黄柏液坐浴治疗效果更佳,可降低炎症因子表达,减轻疼痛,加快创面愈合,促进肛门括约肌功能恢复,值得临床推广应用。Objective To explore the effect of Compound Huangbai Liquid sitz bath assisted internal mouth sealing surgery on postoperative pain,wound healing and anal sphincter function in patients with high perianal abscess.Methods A total of 150 patients with high perianal abscess admitted to the Second People's Hospital of Yangzhou from June 2020 to June 2023 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 75 cases in each group.The control group received internal mouth closure+potassium permanganate sitz bath treatment,the observation group received internal mouth closure+Compound Huangbai Liquid sitz bath treatment.Wound healing,postoperative pain,anal sphincter function,inflammatory factors(interleukin-6[IL-6],tumor necrosis factor-α[TNF-α],C-reactive protein[CRP]and serum indicators(epidermal growth factor[EGF],serum transforming growth factor-β[TGF-β],vascular endothelial cell growth factor[VEGF])were compared between the two groups.Results The scores of wound edema and fluid seepage in the observation group were lower than those in the control group,the healing time was shorter than that in the control group,and the scores of postoperative pain and anal sphincter function were lower than those in the control group,with statistical significance(P<0.05).There were no significant differences in IL-6,TNF-α,CRP,EGF,TGF-βand VEGF levels between the two groups before surgery(P>0.05).After operation,IL-6,TNF-αand CRP in the two groups were lower than before operation,and observation group was lower than control group,the differences were statistically significant(P<0.05).After operation,the levels of EGF,TGF-βand VEGF in the two groups were higher than those before operation,and the observation group was higher than the control group,the differences were statistically significant(P<0.05).Conclusion Patients with high perianal abscess are better treated with internal sealing surgery and Compound Huangbai Liquid sitz bath,which c
