香砂平胃颗粒治疗消化道疾病疗效和安全性的系统评价和Meta分析

摘　　要：目的对香砂平胃颗粒治疗消化道疾病的疗效及安全性进行系统评价。方法根据相关关键词,检索知网、万方、维普、PubMed、Embase数据库文献。采用Review Manager 5.0软件进行数据分析,计算合并的比值比(OR)或均数差(MD)及其95%置信区间(95%CI),检验异质性,评估发表偏倚。结果最终纳入14篇文献,研究质量较好,偏倚风险较低。Meta分析结果显示,香砂平胃颗粒联合西药治疗消化道疾病的总有效率高于西药单独治疗[95%CI=(3.69,9.28),P<0.001],各研究间无异质性(I2=0.0%,P=0.55)。在临床症状积分上,香砂平胃颗粒联合西药可显著改善餐后饱胀、早饱、上腹痛和上腹灼烧感(P<0.05);在胃肠激素水平上,显著改善胃动素、胃泌素和血浆P物质水平(P<0.001)。另外,香砂平胃颗粒可有效清除幽门螺杆菌(Hp),且不增加不良反应。结论与常规西药相比,香砂平胃颗粒联合西药可显著提高临床总有效率,明显改善临床症状积分、胃肠激素水平,有效清除Hp,不良反应少,安全性好。Objective To evaluate the efficacy and safety of Xiangsha Pingwei granules in the treatment of digestive tract diseases.Methods The CNKI,Wanfang,Weipu,PubMed and Embase databases were searched according to the relevant search terms.Review Manager 5.4 software was used to calculate pooled odd ratio(OR)or mean difference(MD)(95%CI),perform heterogeneity test,and assess publication bias.Results Totally 14 literatures were enrolled,with good quality and low risk of bias.Meta-analysis showed that the total effective rate of Xiangsha Pingwei granules combined with western medicine in treatment of digestive tract diseases was higher than that of western medicine alone[95%CI=(3.69,9.28),P<0.001],with no heterogeneity among studies(I2=0.0%,P=0.55).On the clinical symptom score,Xiangsha Pingwei granules combined with western medicine could significantly improve postprandial fullness,early satiation,epigastric pain and epigastric burning(P<0.05).On the level of gastrointestinal hormones,Xiangsha Pingwei granules could significantly improve the levels of motilin,gastrin and plasma substance P(P<0.001).Furthermore,Xiangsha Pingwei granules can effectively remove Helicobacter pylori(Hp)and did not increase adverse reactions.Conclusion Compared with conventional western medicine,Xiangsha Pingwei granule combined with western medicine can significantly improve total clinical effective rate,clinical symptom score and gastrointestinal hormone level,remove Hp,with less adverse reactions and high safety.
