摘 要:目的探索2型糖尿病患者代谢异常与血糖控制的关系。方法将44例2型糖尿病患者分为血糖控制良好组和血糖欠佳组,分别与健康对照组比较,分析两两组间的甘油三酯、总胆固醇、高密度脂蛋白胆固醇及其亚组分、低密度脂蛋白胆固醇(LDL-C)及载脂蛋白A1、B等临床指标的统计学差异。结果健康对照组44例,2型糖尿病研究组44例,血糖控制良好患者数量是14例,血糖控制欠佳患者数量是30例。与健康对照组比较,2型糖尿病总组的TG、Tch、LDL-C显著升高(P<0.05),血糖控制良好组的TG、Tch、ApoB显著升高(P<0.05),血糖控制欠佳组的TG、Tch、ApoB、LDL-C显著升高(P<0.05);进一步分析血糖控制与脂代谢异常的关系,对比分析血糖控制良好组和血糖欠佳组,前者的TG、Tch、ApoB和LDL-C显著下降(P<0.05),差异具有统计学显著性。结论2型糖尿病患者脂代谢异常与血糖控制之间具有相关性,血糖的控制对血脂的改善有一定的影响,不同血糖控制程度与血脂变化密切相关。提示血糖控制对血脂代谢有明显影响。Objective To explore the relationship between metabolic abnormalities and glycemic control in patients with type 2 diabetes.Methods A total of 44 patients with type 2 diabetes mellitus were divided into good glycemic control group and poor glycemic group,and the statistical differences of triglycerides,total cholesterol,high-density lipoprotein cholesterol and its subcomponents,low-density lipoprotein cholesterol(LDL-C),apolipoprotein A1 and B were analyzed between the two groups.Results There were 44 cases in healthy control group and 44 cases in type 2 diabetes research group,including 14 cases in the group with good blood glucose control and 30 cases in the group with poor blood glucose control.Compared with the healthy control group,TG,Tch,LDL-C were significantly increased(P<0.05)in the total group of type 2 diabetes,TG,Tch,and ApoB in the group with good blood glucose control(P<0.05),and TG,Tch,ApoB,and LDL-C in the group with poor glycemic control(P<0.05).The relationship between blood glucose control and abnormal lipid metabolism was further analyzed,and TG,Tch,ApoB and LDL-C decreased significantly(P<0.05)in the group with good blood glucose control and the group with poor blood glucose control,and the difference was statistically significant.Conclusion There is a correlation between abnormal lipid metabolism and blood glucose control in patients with type 2 diabetes,and the control of blood glucose has a certain impact on the improvement of blood lipids,and different degrees of blood glucose control are closely related to blood lipid changes.It suggests that glycemic control has a significant impact on lipid metabolism.
