透明质酸钠凝胶联合雌二醇片/雌二醇地屈孕酮片预防人工流产术后宫腔粘连及对月经的影响

摘　　要：目的探讨透明质酸钠凝胶联合雌二醇片/雌二醇地屈孕酮片对人工流产患者术后宫腔粘连的预防及月经恢复情况的影响。方法选取2019年6月至2022年10月在丽水市中医院行人工流产术的286例患者为研究对象。采用随机数字表法分为联合组和对照组,各143例。对照组予以雌二醇片/雌二醇地屈孕酮片治疗,联合组在对照组基础上予以透明质酸钠凝胶治疗。比较两组患者术后恢复情况、宫腔粘连发生情况、第1次月经情况、随访期内月经情况以及并发症发生情况。结果联合组患者腹痛持续时间、阴道流血时间均短于对照组,阴道流血量少于对照组,子宫内膜厚度大于对照组,差异均有统计学意义(均P<0.01)。联合组患者术后宫腔粘连总发生率为4.90%,低于对照组的12.59%,差异有统计学意义(P=0.021)。联合组患者月经复潮时间短于对照组,经期时间长于对照组,月经量多于对照组,差异均有统计学意义(均P<0.01)。联合组患者术后随访期内平均月经周期短于对照组,平均经期时间长于对照组,平均月经量多于对照组,差异均有统计学意义(均P<0.01)。随访期内,联合组并发症总发生率为3.50%,低于对照组的10.50%,差异有统计学意义(P=0.020)。结论透明质酸钠凝胶联合雌二醇片/雌二醇地屈孕酮片可增强人工流产术患者术后子宫内膜修复以及月经恢复效果,同时提高对术后宫腔粘连等并发症的预防效果。Objective To analyze the effect of sodium hyaluronate gel combined with estradiol/estradiol dydrogesterone(Femoston)tablets on the prevention of intrauterine adhesion and menstrual recovery after induced abortion.Methods A total of 286 patients who underwent induced abortion in Lishui Hospital of Traditional Chinese Medicine from June 2019 to October 2022 were enrolled in the study.Patients were randomly divided into two groups with 143 patients in each group.The control group was treated with Femoston tablets,and the study group was treated with sodium hyaluronate gel combined with Femoston.Postoperative recovery,intrauterine adhesion,first menstruation,menstruation during follow-up and the incidence of complications were compared between the two groups.Results The duration of abdominal pain and vaginal bleeding in the study group were shorter than those in the control group,the volume of vaginal bleeding was less than that in the control group,and the endometrial thickness was higher than those in the control group(all P<0.01).The total incidence of postoperative intrauterine adhesion in the study group was lower than that in the control group(4.90%vs.12.59%,P=0.021).The time of menstruation recovery in the study group was shorter than that in the control group,the menstrual period was longer than that in the control group,and the menstrual volume was more than that in the control group(all P<0.01).During the postoperative follow-up period,the average menstrual cycle of the patients in the study group was shorter than that in the control group,the average menstrual duration was longer than that in the control group,and the average menstrual volume was more than that of the control group(all P<0.01).The incidence rate of postoperative complications in the study group was lower than that in the control group(3.50%vs.10.50%,P=0.020).Conclusion Sodium hyaluronate gel combined with Femoston tablets can promote endometrial repair and menstruation recovery after induced abortion with lower incidence of postoperative c
