2020—2022年某地暗娼艾滋病高危行为干预效果分析

摘　　要：目的 分析2020—2022年某地暗娼(FSWs)艾滋病(AIDS)哨点监测结果,为该地今后工作指引方向。方法 采取分层整群抽样的方法进行现场调查,利用描述性研究比较历年高危行为干预工作指标,利用多因素回归分析暗娼高危行为的影响因素。结果 1200名暗娼艾滋病防控相关知识知晓率为94.83%。2020—2022年该人群HIV检测意愿呈下降趋势(P<0.05),接受艾滋病宣传和咨询及同伴教育呈上升趋势,各年最近1次性行为安全套使用情况呈上升趋势。该人群艾滋病、梅毒和丙肝的检出比例较低,愿意接受宣传和咨询(OR=1.945,95%CI:1.274~2.971)、来本地时间(OR=0.668,95%CI:0.454~0.983)、知晓安全套的防控作用(OR=9.390,95%CI:3.848~22.914)等自变量是安全套使用的影响因素。结论 该地高危行为干预工作重点应转向提升暗娼人群的安全套推广使用工作上来,重点提升该人群的艾滋病相关知识宣传咨询教育和安全套防控作用知晓等工作。Objective To analyze AIDS sentinel surveillance data among female sex workers(FSWs)in a certain area from 2020 to 2022,and to provide guidance for future intervention work in this area.Methods A stratified cluster sampling method was used for on⁃site investigation.Descriptive study was used to compare high⁃risk behaviors over the years,and multivariate regression analysis was used to analyze the influencing factors associated with high⁃risk behaviors among FSWs.Results The awareness rate of AIDS prevention and control related knowledge among 1200 FSWs was 94.83%.From 2020 to 2022,the willingness of HIV testing showed a downward trend(P<0.05).The detection rates of AIDS,syphilis and hepatitis C in this population was relatively low,and independent variables such as willingness to accept publicity and consultation(OR=1.945,95%CI:1.274-2.971),duration of living local area(OR=0.668,95%CI:0.454-0.983)and awareness of condom prevention and control(OR=9.390,95%CI:3.848-22.914)were influencing factors associated with condom use.Conclusion The key intervention activity on high⁃risk behavior in this area should be shifted to the promotion on condom use,counseling and education of AIDS related knowledge and the awareness of condom prevention and control among FSWs.
