临床药师“传帮带”提高县级医院合理用药水平的效果分析

摘　　要：目的探讨基于处方点评的“传帮带”实践对于提高县级医院药师的专业水平和促进合理用药的效果。方法选取2022年上半年某县级中医院药师点评的1800张处方、下半年三级医院临床药师带领下点评的1800张处方作为研究对象,比较两组合理用药情况。结果2022年下半年三级医院临床药师干预后,平均每张处方用药品种数、国家基本药物使用率、静脉给药使用率、门诊抗菌药物使用率和门诊每次平均药品费用等指标均得到改善。上半年不合理处方265张(14.72%),下半年不合理处方143张(7.94%),不规范处方、不适宜处方明显减少(P<0.05),超常处方虽减少,但无统计学差异(P>0.05)。结论三级医院临床药师通过传授知识和管理经验,实现了对县级医院的“传帮带”,显著提高县级医院药师的知识水平和点评能力,提高其药事管理水平,推动合理用药工作持续改进和提升。Objective Explore the effects of"mentorship"practice based on prescription evaluation on enhancing the professional competence of pharmacists in county hospitals and promoting rational use of medications.Methods A total of 1800 prescriptions evaluated by county-level hospital pharmacists in the first half of 2022 were compared with 1800 prescriptions evaluated under the guidance of clinical pharmacists from tertiary hospitals in the second half.The rational drug use status was compared between the two groups.Results After the intervention of clinical pharmacists in tertiary hospitals in the second half of 2022,indicators such as the average number of drug varieties per prescription,the utilization rate of national essential drugs,the utilization rate of intravenous drug administration,the utilization rate of antibacterial drugs in outpatient clinics and the average drug cost per outpatient clinic were improved.There were 265 irrational prescriptions in the first half of the year(14.72%),143 irrational prescriptions in the second half(7.94%),irregular prescriptions and inappropriate prescriptions decreased significantly(P<0.05),and abnormal prescriptions decreased,but there was no statistical difference(P>0.05).Conclusions Clinical pharmacists in tertiary hospitals can"pass along"to county-level hospitals by imparts knowledge and management experience,significantly improve the knowledge level and comment ability of pharmacists in county-level hospitals,improve their pharmaceutical management level,and promote the continuous improvement and promotion of rational drug use.
