芪丹军蛭汤整方随证施量策略在糖尿病肾病治疗中的运用

摘　　要：方药剂量的调整包括单味药剂量的调整和整方剂量的调整等,对于药少而精、功效专一的方剂,多采用整方调量的方式,整方调量也要遵循随证施量策略。糖尿病肾病是常见的糖尿病微血管并发症之一,该疾病早中期,患者往往有蛋白尿,尿微量白蛋白肌酐比(ACR)升高等表现,芪丹军蛭汤是仝小林院士治疗糖尿病肾病的靶方,其调量的指征是尿微量白蛋白和尿微量白蛋白肌酐比,临证应用时多根据指征的变化对整方剂量进行调整,在确保用药安全的基础上可取得最佳疗效。The drug dosage adjustment in prescriptions includes the dosage adjustment of a single herb and the dosage adjustment of a whole prescription.For those prescriptions with fewer but more precise drugs and specific effects,the dosage adjustment of a whole prescription is mostly adopted,and the adjustment should be done on the basis of syndrome differentiation.Diabetic kidney disease is one of the common microvascular complications of diabetes.In its early and middle stages,patients often undergo proteinuria and increased ACR.Qidan Junzhi Decoction is the target prescription administered by academician Tong Xiaolin in the treatment of diabetic nephropathy,and the indications of its dosage adjustment are urinary microalbumin and ACR.Therefore,in its clinical application,the dosage of the whole prescription is adjusted according to the changes of indications in case the best curative effect can be achieved and the safety of medication can be ensured.
