摘　　要：仝小林院士认为,慢性心力衰竭主要责之于气、血、水,气不足,则动力乏源,血必瘀滞,血不利则为水,故心衰之责,无非气、血、水,与西医强心、利尿、扩血管之治则相同。本文从态靶辨治理论出发治疗难治性心力衰竭病案一则,此患者以虚态为主,瘀血、水停为靶,故以延命汤合真武汤态靶同调,取得了良好临床疗效。将中医传统临床诊疗思路与现代医学疑难问题有机结合,调态与打靶同施,标本兼顾,为临床治疗慢性心力衰竭提供了新的诊疗思路。Academician Tong Xiaolin believes that chronic heart failure is mainly caused by insufficient qi,blood stasis,and water retention,which may lead to a lack of power in the heart.His view on the pathogenesis of the disease is consistent with the treatment principle of strengthening the heart,promoting diuresis,and vasodilation in the treatment of chronic heart failure in western medicine.This article introduces a medical case of refractory heart failure treated by academician Tong Xiaolin based on the theory of state-target differentiation,in which the patient is mainly manifested as a deficiency state,with blood stasis and water retention as targets.Tong Xiaolin regulates the state and the targets with the combination of Yanming Decoction and Zhenwu Decoction simultaneously,achieving a good clinical efficacy.In this case,Academician Tong Xiaolin integrates traditional Chinese medicine clinical diagnosis and treatment ideas with critical cases in modern medicine,regulating states and targets at the same time,treating both manifestations and the root cause of the disease,providing a new diagnosis and treatment approach for the clinical treatment of chronic heart failure.
