清热渗湿汤辅助治疗良性前列腺增生PVP术后临床研究

摘　　要：目的:观察清热渗湿汤辅助治疗良性前列腺增生(BPH)经尿道选择性绿激光前列腺汽化术(PVP)术后的临床疗效。方法:采用随机数字表法将150例择期行PVP的BPH患者分为观察组与对照组各75例。2组均给予PVP治疗,对照组术后加用盐酸坦洛新缓释片治疗,观察组在对照组基础上联合清热渗湿汤治疗。比较2组治疗前后中医证候积分、国际前列腺症状评分(IPSS评分)、排尿功能、生活质量(QOL)评分、性激素水平;比较2组临床疗效及不良反应发生率。结果:观察组总有效率92.00%,高于对照组78.67%(P<0.05)。2组治疗后中医证候积分、IPSS评分、QOL评分均较治疗前降低(P<0.05),且观察组治疗后上述指标低于对照组(P<0.05)。2组治疗后日间排尿次数、夜间排尿次数、尿急次数、残余尿量(PVR)、促黄体生成素(LH)、雌二醇(E_(2))水平均较治疗前降低(P<0.05),最大尿流率(Qmax)、睾酮(T)、催乳素(PRL)水平均升高(P<0.05);观察组治疗后日间排尿次数、夜间排尿次数、尿急次数、PVR、LH、E_(2)水平低于对照组(P<0.05);Qmax、T、PRL水平高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清热渗湿汤辅助治疗PVP术后BPH疗效确切,能改善患者排尿功能与性激素水平,提高生活质量,安全性较高。Objective:To observe the clinical effect of adjuvant treatment of Qingre Shenshi Decoction on benign prostatic hyperplasia(BPH) after green-light laser photoselective vaporisation of the prostate(PVP). Methods: A total of 150 cases of BPH patients who underwent PVP were divided into the the observation group and the control group by random number table, with 75 cases in each group. Both groups were treated with PVP;after operation,the control group was additionally treated with Tylosine Hydrochloride Sustained Release Tablets,and the observation group was additionally treated with Qingre Shenshi Decoction. Before and after treatment,the traditional Chinese medicine(TCM) syndrome scores,International prostate symptom score(IPSS score),urinary function,Quality of life(QOL) scores,and sex hormone levels were compared in two groups. The clinical effects and rate of side reactions were analyzed.Results:The total effective rate was 92.00% in observation group,higher than that of 78.67% in control group(P<0.05). After treatment, the scores of TCM syndromes, IPSS, and QOL in two groups were decreased(P<0.05),and the above indexes in observation group were lower compared with control group(P<0.05). After treatment, the daytime urination frequency, nighttime urination frequency, urgent urination frequency,post-void residual volume(PVR),luteinizing hormone(LH),and estradiol(E_(2)) levels in the two groups were decreased(P<0.05),and the maximum urinary flow rate(Qmax),testosterone(T),and prolactin(PRL) levels were increased(P<0.05);the daytime urination frequency,nighttime urination frequency, urgent urination frequency, PVR, LH, and E2levels in observation group were lower(P<0.05),and the levels of Qmax,T,and PRL were higher than those in the control group(P<0.05). The rate of side reactions was no difference in two groups(P>0.05).
