中药熏蒸联合神经阻滞治疗腰椎间盘突出症临床研究

摘　　要：目的:观察中药熏蒸联合神经阻滞对腰椎间盘突出症(LDH)患者的临床疗效。方法:选取120例LDH患者,采用随机数字表法分为观察组和对照组各60例。观察组采用中药熏蒸联合神经阻滞疗法,对照组采用单纯神经阻滞疗法。比较2组临床疗效,并比较2组治疗前后中医证候积分、Oswestry功能障碍指数(ODI)评分、视觉模拟评分法(VAS)评分、日本骨科协会评估治疗(JOA)评分。结果:治疗后,观察组总有效率91.67%,高于对照组78.33%(P<0.05)。治疗后,2组腰腿酸痛、腰部板硬、局部压痛等中医证候积分均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组ODI、VAS评分均降低(P<0.05),且观察组低于对照组(P<0.05);2组JOA评分均升高(P<0.05),且观察组高于对照组(P<0.05)。结论:中药熏蒸联合神经阻滞疗法在减轻LDH患者腰腿酸痛、缓解腰部板硬、减轻局部压痛、改善腰椎活动功能等方面优于单纯神经阻滞疗法。Objective:To observe the clinical effect of Chinese herbal fumigation combined with nerve block on patients with lumbar disc herniation(LDH). Methods:A total of 120 cases of LDH patients were selected and divided into the observation group and the control group according to the random number table method,with 60 cases in each group. The observation group was treated with Chinese herbal fumigation combined with nerve block therapy, and the control group was simply given nerve block therapy. The clinical effects were compared between the two groups;traditional Chinese medicine(TCM) syndrome scores, Oswestry Disability Index(ODI) scores, Visual Analogue Scale(VAS) scores, and Japanese Orthopaedic Association(JOA) scores were compared before and after treatment between the two groups.Results:After treatment,the total effective rate was 91.67% in the observation group,higher than that of78.33% in the control group(P<0.05). After treatment,the scores of TCM syndrome such as soreness in waist and legs,lumbar stiffness and local tenderness in the two groups were decreased(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05). After treatment,the scores of ODI and VAS in the two groups were decreased(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05);JOA scores in the two groups were increased(P<0.05),and the score in the observation group was higher than that in the control group(P<0.05). Conclusion: When compared with simple nerve block therapy, Chinese herbal fumigation combined with nerve block therapy has a better effect on alleviating soreness and pain in waist and legs,relieving lumbar stiffness and local tenderness and improving lumbar movement function in LDH patients.
