天枢穴中药超声导入法联合盆底生物反馈治疗顽固性便秘临床研究

摘　　要：目的:观察天枢穴中药超声导入法联合盆底生物反馈治疗顽固性便秘的疗效。方法:选取82例顽固性便秘患者为研究对象,按随机数字表法分为对照组及研究组各41例。对照组采用盆底生物反馈治疗,研究组采用天枢穴中药超声导入法联合盆底生物反馈治疗。比较2组临床疗效,比较2组治疗前后中医证候积分、直肠肛管压力[肛门括约肌静息压力(RASP)、肛管括约肌最大收缩压力(SASP)、直肠最大耐受阈值(MRST)及直肠排便阈值(CRS)]测定值的变化。结果:治疗1个月后,研究组临床疗效总有效率为97.56%,对照组为85.37%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分(主症、次症、总分)均较治疗前下降(P<0.05),研究组各中医证候积分均低于对照组(P<0.05)。治疗1个月后,2组RASP、SASP、CRS指标值均较治疗前下降(P<0.05),MRST指标值均较治疗前升高(P<0.05);研究组RASP、SASP、CRS指标值均低于对照组(P<0.05),MRST指标值高于对照组(P<0.05)。结论:天枢穴中药超声导入法联合盆底生物反馈治疗顽固性便秘临床疗效较好,能缓解症状,改善直肠肛管功能。Objective:To observe the curative effect of the therapy of Chinese medicine delivered by sonophoresis at Tianshu Point(ST 25)combined with pelvic floor biofeedback on refractory constipation.Methods:A total of 82 patients with refractory constipation were selected as the study subjects and divided into the control group and the study group,with 41 cases in each group.The control group was given pelvic floor biofeedback for treatment,and the study group was treated with Chinese medicine delivered by sonophoresis at Tianshu Point combined with pelvic floor biofeedback.The clinical effects,and the changes of traditional Chinese medicine syndrome scores,and measured values of anorectal pressure indexes[resting anal sphincter pressure(RASP),maximum systolic anal sphincter pressure(SASP),maximum rectal tolerance threshold(MRST)and rectal defecation threshold(CRS)]were compared before and after treatment between the two groups.Results:After treatment of one month,the total clinical effective rate was 97.56%in the study group and 85.37%in the control group,the difference being significant(P<0.05).After treatment,the TCM syndrome scores(the scores of primary and secondary symptoms and the total scores)in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the study group were lower than those in the control group(P<0.05).After treatment of one month,the values of RASP,SASP and CRS in the two groups were decreased when compared with those before treatment(P<0.05),and the MRST values were increased when compared with those before treatment(P<0.05);the values of RASP,SASP and CRS in the study group were lower than those in the control group(P<0.05),and the MRST value Was higher than that in the control group(P<0.05).Conclusion:The therapy of Chinese medicine delivered by sonophoresis at Tianshu Point combined with pelvic floor biofeedback has a good clinical effect on refractory constipation,which can mitigate the symptoms and improve the anorectal function.
