内关、足三里、合谷三穴经皮电刺激对老年胃癌术后胃肠功能恢复的影响

摘　　要：目的:观察内关、足三里、合谷三穴经皮电刺激对老年胃癌术后胃肠功能恢复的影响。方法:选取180例老年胃癌术后患者,按随机数字表法分为常规组与电刺激组各90例。均给予康复治疗,电刺激组给予真内关、足三里、合谷三穴经皮电刺激,常规组给予假内关、足三里、合谷三穴经皮电刺激。观察2组临床疗效及胃肠道功能恢复(肠鸣音恢复正常时间、首次肛门排气、首次排便时间)情况,比较2组术前2 d及术后3 d时的兴奋性胃肠激素指标[胃动素(MTL)、胃泌素(GAS)、P物质(SP)]、抑制性胃肠激素[血管活性肠肽(VIP)、生长抑素(SS)、胆囊收缩素(CCK)]指标值的变化。结果:电刺激组临床疗效总有效率为68.89%,对照组为53.33%,2组比较,差异有统计学意义(P<0.05)。电刺激组肠鸣音恢复正常时间、首次肛门排气时间、首次排便时间均短于常规组(P<0.05)。术后3 d,2组MTL、GAS水平均较术前下降(P<0.05),SP水平均较术前增加(P<0.05);电刺激组MTL、GAS、SP水平均高于常规组(P<0.05)。术后3 d,2组VIP、SS水平均较术前下降(P<0.05),CCK水平均较术前升高(P<0.05);电刺激组VIP水平低于常规组(P<0.05),SS、CCK水平高于常规组(P<0.05)。结论:内关、足三里、合谷三穴经皮电刺激可促进老年胃癌术后胃肠功能恢复,改善抑制性胃肠激素。Objective:To observe the effect of percutaneous electrical stimulation at Neiguan point(PC6),Zusanli point(ST36)and Hegu point(LI4)on the recovery of gastrointestinal function in elderly patients after surgery for gastric cancer.Methods:A total of 180 elderly patients after surgery for gastric cancer were selected and divided into the conventional group and the electrical stimulation group according to random number table method,with 90 cases in each group.The two groups were given rehabilitation treatment;the electrical stimulation group was given percutaneous electrical stimulation at authentic Neiguan,Zusanli and Hegu points,and the conventional group was given percutaneous electrical stimulation at false Neiguan,Zusanli and Hegu points.The clinical effects and gastrointestinal function recovery,including time of bowel sound returning to normal,first anal farting time and first defecation time,were observed in the two groups.The changes of values of excitatory gastrointestinal hormones,including motilin(MTL),gastrin(GAS)and substance P(SP),and inhibitory gastrointestinal hormones,including vasoactive intestinal peptide(VIP),somatostatin(SS)and cholecystokinin(CCK),Were compared between the two groups 2 days before surgery and 3 days after surgery.Results:The total effective rate Was 68.89%in the electrical stimulation group and 53.33%in the control group,the difference being significant(P<0.05).The time of bowel sound returning to normal,first anal farting time and first defecation time in the electrical stimulation group were shorter than those in the conventional group(P<0.05).Three days after surgery,the MTL and GAS levels in the two groups were decreased when compared with those before surgery(P<0.05),and SP levels were increased when compared with those before surgery(P<0.05);the levels of MTL,GAS and SP in the electrical stimulation group were higher than those in the conventional group(P<0.05).Three days after surgery,the levels of VIP and SS in the two groups were decreased when compared with those bef
