中西医治疗腹膜透析患者微炎症状态概况

摘　　要：腹膜透析(peritoneal dialysis, PD)是慢性肾脏病终末期的肾脏替代治疗方法之一,长期PD会导致患者微炎症状态等并发症发生。西医主要采用血管紧张素受体阻滞药(angiotensin receptor blockers, ARB)类、他汀类、左卡尼汀(肉碱)等药物治疗;中医药治疗主要包括中成药、中药复方、中西医结合以及其他治疗等。目前,研究存在的问题是:⑴主要为临床试验研究,缺乏高质量的基础研究;(2)临床研究样本量较小,缺乏多中心、大样本、随机的对照研究,治疗周期相对较短,缺乏对其远期疗效资料的统计分析;(3)缺乏公认的证型标准和诊疗规范,缺乏统一的结果指标。今后,应加强该领域的基础研究,充分利用现代科学技术来阐明疾病的发病机制,为中医药治疗PD微炎症状态提供更多的科学依据,完善临床研究相关设计,提供更加合理的研究成果,制定更加规范的诊疗标准,在临床治疗中,根据实际情况去选择合适的治疗手段,发挥出中西医结合治疗的优势,完善中西医结合诊疗体系,提高腹膜透析患者生存率及生活质量。Peritoneal dialysis(PD)is one of the renal replacement therapies for end-stage chronic kidney disease(CKD),and long-term PD can lead to complications such as micro-inflammation state.Western medicine mainly uses ARB,statins,L-carnitine and other drugs for treatment,while TCM treatments mainly include Chinese patent medicines,TCM compounds,integrated TCM and Western medicine.At present,the problems existing in the research are as follows:(1)There is mainly clinical trial research,with a lack of high-quality basic research.(2)There is small sample size of clinical research,with a lack of multi-center,large sample,randomized controlled study,relatively short treatment cycle,and statistical analysis of long-term efficacy data.(3)There is a lack of recognized syndrome standard and diagnosis and treatment specification,and unified outcome indicators.In the future,it is necessary to strengthen the basic research in this field,make full use of modern science and technology to clarify the pathogenesis of the disease,provide more scientific basis for the treatment of PD micro-inflammation state by TCM,improve the clinical research design,provide more reasonable research results,and formulate more standardized diagnosis and treatment standards.In clinical treatment,appropriate treatments need to be selected based on the actual situation,the advantages of integrating TCM and Western medicine treatment should be fully utilized,and the relevant diagnosis and treatment systems should be improved to improve the survival rate and quality of life of PD patients.
