摘  要:目的:探讨线粒体耦联因子-6(CF-6)、可溶性生长刺激表达基因2蛋白(sST2)、生长分化因子-15(GDF-15)在冠心病(CHD)合并心力衰竭(HF)患者中的水平及与左室射血分数(LVEF)的相关性。方法:选择2020年1月-2021年1月我院收治的CHD合并HF患者134例为研究组,另随机选择同期于我院进行体检的健康志愿者120例为对照组。对比两组患者CF-6、sST2、GDF-15表达水平;比较不同心功能分级、不同LVEF患者CF-6、sST2、GDF-15水平及与LVEF的相关性。结果:与对照组相比,研究组CF-6[(241.35±24.12)ng/L比(523.98±55.23)ng/L]、sST2[(0.23±0.04)μg/L比(1.74±0.19)μg/L]、GDF-15[(401.23±53.26)pg/ml比(1732.32±443.99)pg/ml]水平均显著升高,LVEF[(58.22±8.30)%比(47.40±7.22)%]显著降低(P均=0.001)。与NYHAⅡ级组相比,Ⅲ级、Ⅳ级组sST2、GDF-15水平显著升高,且Ⅳ级组的显著高于Ⅲ级组的(P<0.05或<0.01)。与射血分数保留型心力衰竭(HFpEF)组相比,射血分数中间值心力衰竭(HFmrEF)组、射血分数减低型心力衰竭(HFrEF)组sST2、GDF-15水平显著升高,且HFrEF组GDF-15水平显著高于HFmrEF组(P均=0.001)。Pearson相关分析显示,CF-6、sST2、GDF-15水平与LVEF呈显著负相关(r=-0.659、-0.664、-0.592,P均=0.001)。结论:CF-6、sST2、GDF-15水平在冠心病合并心力衰竭患者中显著升高,且三者与左室射血分数显著负相关,可以用于反映心脏功能。Objective:To explore levels of mitochondrial coupling factor-6(CF-6),soluble growth stimulating expression gene 2 protein(sST2)and growth differentiation factor-15(GDF-15)in patients with coronary heart disease(CHD)and heart failure(HF)and their correlation with left ventricular ejection fraction(LVEF).Methods:A total of 134 CHD+HF patients who were admitted to our hospital from January 2020 to January 2021 were selected as study group,and another 120 healthy volunteers who simultaneously underwent physical examination in our hospital were randomly selected as control group.Expression levels of CF-6,sST2 and GDF-15 were compared between the two groups,among patients with different NYHA class and different LVEF.Their correlation with LVEF was analyzed.Results:Compared with the control group,there were significant rise in levels of CF-6[(241.35±24.12)ng/L vs.(523.98±55.23)ng/L],sST2[(0.23±0.04)μg/L vs.(1.74±0.19)μg/L]and GDF-15[(401.23±53.26)pg/ml vs.(1732.32±443.99)pg/ml],and significant reduction in LVEF[(58.22±8.30)%vs.(47.40±7.22)%]in study group(P=0.001 all).Compared with NYHA class Ⅱ group,there were significant rise in sST2 and GDF-15 levels in class Ⅲ and class Ⅳ group,and those of class Ⅳ group were significantly higher than those of class Ⅲ group,P<0.05 or<0.01.Compared with heart failure with preserved ejection fraction(HFpEF)group,there were significant rise in sST2 and GDF-15 levels in heart failure with medium-reduced ejection fraction(HFmrEF)and heart failure with reduced ejection fraction(HFrEF)group,and GDF-15 level in HFrEF group was significantly higher than that of HFmrEF group,P=0.001 all.Pearson correlation analysis indicated that levels of CF-6,sST2 and GDF-15 were significant inversely correlated with LVEF(r=-0.659,-0.664,-0.592,P=0.001 all).Conclusion:Levels of CF-6,sST2 and GDF-15 significantly rise in patients with coronary heart disease and heart failure,and all three are significant inversely correlated with LVEF,which could be used to reflect heart function.
