摘  要:目的:探讨β受体阻滞剂对快速型心律失常患者心肺运动能力的影响。方法:纳入2021年1月~2023年6月于复旦大学附属华东医院心血管内科就诊并确诊快速型心律失常的患者92例,根据患者心肺运动试验(CPET)前是否服用β受体阻滞剂分为对照组(45例,未使用β受体阻滞剂)及药物组(47例,使用琥珀酸美托洛尔缓释片)。收集两组患者的基线资料,比较CPET指标的差异。结果:与对照组比较,药物组静息心率[(78.73±11.46)次/min比(73.72±11.21)次/min]、峰值心率[(125.69±18.39)次/min比(115.15±15.94)次/min]、峰值摄氧量[(1.36±0.31)L/min比(1.21±0.26)L/min]、峰值摄氧量占预计值百分比[(75.38±15.73)%比(67.96±14.22)%]、峰值功率[(104.73±22.70)W比(94.98±20.57)W]、峰值代谢当量[(5.73±1.27)比(5.13±1.15)]均显著降低,P<0.05或<0.01。Pearson相关分析显示,药物组峰值摄氧量与峰值心率呈显著正相关(r=0.453,P=0.001)。结论:β受体阻滞剂可降低快速型心律失常患者心肺运动能力,这可能与β受体阻滞剂的负性心率作用有关。Objective:To investigate the effect of β-receptor blockers on cardiopulmonary exercise capacity in patients with tachyarrhythmia.Methods:A total of 92 patients,who visited Department of Cardiology,Affiliated Huadong Hospital of Fudan University from January 2021 to June 2023 and were diagnosed as tachyarrhythmia,were enrolled.According toβreceptor blocker usage before cardiopulmonary exercise test(CPET)or not,they were divided into control group(n=45,didn't useβreceptor blocker)and drug group(n=47,used metoprolol succinate sustained-release tablet).The baseline data of the two groups were collected,and CPET indexes were compared between two groups.Results:Compared with the control group,there were significant reductions in resting heart rate[(78.73±11.46)beats/min vs.(73.72±11.21)beats/min],peak heart rate[(125.69±18.39)beats/min vs.(115.15±15.94)beats/min],peak oxygen uptake[(1.36±0.31)L/min vs.(1.21±0.26)L/min],peak oxygen uptake as a percentage of predicted[(75.38±15.73)%vs.(67.96±14.22)%],peak power[(104.73±22.70)W vs.(94.98±20.57)W]and peak metabolic equivalent[(5.73±1.27)vs.(5.13±1.15)]in the drug group,P<0.05 or<0.01.Pearson correlation analysis indicated that there was a significant positive correlation between peak oxygen uptake and peak heart rate in the drug group(r=0.453,P=0.001).Conclusion:β-receptor blockers can reduce cardiopulmonary exercise capacity in patients with tachyarrhythmia,which may be associated with the negative heart rate effects ofβ-receptor blockers.
