温针灸联合腹针对脑卒中后下肢痉挛性瘫痪患者功能恢复及血清血管活性物质水平的影响

摘　　要：目的研究温针灸联合腹针对脑卒中后下肢痉挛性瘫痪患者的功能恢复及血清血管活性物质水平的影响。方法选取2020年5月—2022年5月期间接诊的120例脑卒中后下肢痉挛性瘫痪患者作为研究对象。按随机数表法随机分为研究组与对照组各60例。对照组采用常规治疗方法,研究组采用温针灸联合腹针进行治疗。两组患者均连续治疗5周,记录并比较两组患者在治疗前后Fugl-Meyer运动功能量表(FMA)评分、改良Barthel指数(MBI)评分、Berg平衡量表(BBS)评分、痉挛指数评分、10 m步行时间、血管内皮生长因子(VEGF)、血管紧张素-1(Ang-1)水平。结果治疗前,两组患者的FMA评分、MBI评分、BBS评分相比,差异无统计学意义(P>0.05),治疗后,两组患者的FMA评分、MBI评分、BBS评分均升高,且研究组明显高于对照组,差异有统计学意义(P<0.05);治疗前,两组患者的痉挛指数评分、10 m步行时间相比,差异无统计学意义(P>0.05),治疗后,两组患者的痉挛指数评分、10m步行时间均降低/缩短,且研究组低/短于对照组,差异有统计学意义(P<0.05);治疗前,两组患者的VEGF、Ang-1水平相比,差异无统计学意义(P>0.05),治疗后,两组患者的VEGF、Ang-1水平均升高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论温针灸联合腹针能有效地促进脑卒中后下肢痉挛性瘫痪患者的功能恢复,这可能与血清血管活性物质水平的升高有关,具有临床应用价值,值得推广。Objective To study the effect of warm acupuncture combined with abdominal acupuncture on functional recovery and serum vasoactive substances in patients with spastic paralysis of lower limbs after stroke.Methods A total of 120 patients with spastic paralysis of lower limbs after stroke who were admitted to the hospital from May 2020 to may 2022 were selected as the research objects.The patients were randomly divided into study group and control group with 60 cases in each group.The control group was treated with routine treatment,and the study group was treated with warm acupuncture combined with abdominal acupuncture.The two groups were treated for 5 weeks.The scores of Fugl Meyer motor function scale(FMA),modified Barthel index(MBI),Berg balance scale(BBS),spasm index,10 m walking time,vascular endothelial growth factor(VEGF)and angiotensin-1(Ang-1)were recorded and compared before and after treatment.Results Before treatment,there was no significant difference in FMA score,MBI score or BBS score between the two groups(P>0.05).After treatment,the FMA score,MBI score and BBS score of the two groups increased,and the scores of the study group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the score of spasticity index or 10 m walking time between the two groups(P>0.05).After treatment,the score of spasticity index and 10 m walking time of the two groups decreased/shortened and those of the study group were lower/shorter than those of the control group.The difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of VEGF or Ang-1 between the two groups(P>0.05).After treatment,the levels of VEGF and Ang-1 in the two groups increased,and the levels in the study group were higher than those in the control group(P<0.05).Conclusion Warm acupuncture combined with abdominal acupuncture can effectively promote the functional recovery of patien
