摘　　要：目的探讨不同手术入路治疗男性复杂骨盆骨折的疗效及对性功能的影响。方法77例男性复杂骨盆骨折患者,根据手术方式不同分为改良Stoppa入路组(20例)、髂腹股沟入路组(31例)、前后联合入路组(26例),术后1年采用国际勃起功能问卷5(IIEF-5)评估性功能,采用Majeed评分评估患者骨盆功能,观察术后并发症发生情况。结果改良Stoppa入路组、髂腹股沟入路组及前后联合入路组IIEF-5评分、勃起功能障碍发生率及术后并发症发生率比较差异无统计学意义(P>0.05)。改良Stoppa入路组Majeed评分高于前后联合入路组[(88.15±9.54)分vs.(79.58±11.89)分](P<0.05)。结论对于男性复杂骨盆骨折,不同手术入路对性功能的影响无明显差异;在疗效方面,改良Stoppa入路优于前后联合入路。Objective To explore the efficacy of different surgical approaches and their effect on sexual function in the treatment of complex pelvic fracture in males.Methods A total of 77 male patients with complex pelvic fracture were divided into three groups of A(using modified Stoppa approach,20 cases),B(using ilioinguinal approach,31 cases)and C(using combined anterior and posterior approach,26 cases).The international index of erectile function 5(IIEF-5)was used to evaluate sexual function one year after surgery.The pelvic function was evaluated by Majeed score.The postoperative complications were observed.Results There were no significant differences in IIEF-5 score,incidence of erectile dysfunction and postoperative complications among the three groups(P>0.05).The IIEF-5 score was higher in group A than that in group C[(88.15±9.54)points vs.(79.58±11.89)points](P<0.05).Conclusion There is no significant difference in the effect of different surgical approaches on sexual function in the treatment of complex pelvic fracture in males.The modified Stoppa approach has better therapeutic effect than combined anterior and posterior approach.
