摘　　要：慢性心力衰竭是各种心脏疾病的最终转归,被称为“心脏病最后的战场”,其高死亡率、高住院率一直是治疗上的一大难点。西医治疗慢性心衰虽有一定的进展,但仍有一定的局限性,而中医药在改善症状、改善预后、增强患者体质方面具有一定优势,因而被各大医家广泛运用。心力衰竭在中医学上归属为“心衰病”范畴,其病机为本虚标实,心气不足为本,饮停、痰浊、瘀血为标。鲍荣琦在继承前人理论的基础上再加上自身对慢性心衰的理解,从心胃理论出发,认为慢性心衰的主要病机是气虚阳衰,血瘀水停,心胃同病。根本在于心气(阳)不足、脾肾阳虚,水湿、痰饮、瘀血为其次。心胃同病是其特殊的病机。治疗上以补益宗气为主,同时配合理气的方法,达到心胃同治的目的。本文主要论述心胃在生理病理上的相互关联,探讨以心胃同治论治慢性心力衰竭,为中医治疗慢性心衰提供新思路。Chronic heart failure is the ultimate outcome of various heart diseases,known as the"last battlefield of heart disease",and its high mortality rate and high hospitalization rate have always been a major difficulty in treatment.Although western medicine has made some progress in the treatment of chronic heart failure,it still has certain limitations,while traditional Chinese medicine has certain advantages in improving symptoms,improving prognosis and enhancing patients'physical fitness,therefore,it is widely used by major doctors.Heart failure belongs to the category of"heart failure disease"in traditional Chinese medicine,and its pathogenesis is based on false standards,insufficient heart Qi,drinking stoppage,phlegm turbidity,and blood stasis.On the basis of inheriting the theory of his predecessors and his own understanding of chronic heart failure,Bao Rongqi started from the theory of heart and stomach,and believed that the main pathogenesis of chronic heart failure is Qi deficiency and Yang failure,blood stasis and water stoppage,and heart and stomach disease.The root cause lies in insufficient heart Qi(Yang),spleen and kidney Yang deficiency,followed by moisture,phlegm drinking and blood stasis.Heart and stomach disease is its special pathogenesis.In terms of treatment,it is mainly based on tonifying Zongqi,and at the same time combining reasonable Qi methods to achieve the purpose of same treatment of heart and stomach treatment.This paper mainly discussed the physiological and pathological correlation between heart and stomach,discussed the treatment of chronic heart failure with the same treatment of heart and stomach,and provided new ideas for the treatment of chronic heart failure in traditional Chinese medicine.
