超声引导下喉上神经内支阻滞复合静脉全麻用于支撑喉镜下声带息肉摘除术的效果观察

摘　　要：目的探究超声引导下喉上神经内支阻滞复合静脉全麻用于支撑喉镜下声带息肉摘除术的效果。方法选择2020年3月至2022年5月期间在本院行支撑喉镜下声带息肉摘除术的72例患者为研究对象,随机分成对照组和观察组,每组36例。对照组实施静脉全麻,观察组实施超声引导下喉上神经内支阻滞复合静脉全麻,观察两组不同时点MAP和HR变化,比较两组术后2h、4h、8h、12h、24h、48h的VAS评分,统计两组术后不良反应。结果与T0时点相比,对照组在T1~T5时点的MAP、HR均明显上升(P<0.05),观察组在T1~T5时点的MAP、HR均无明显变化(P>0.05);观察组在T1~T5时点的MAP、HR均明显低于对照组(P<0.05)。观察组术后2h、4h、8h、12h、24h、48h的VAS评分均低于对照组(P<0.05)。对照组术后不良反应发生率为16.67%,观察组未见不良反应(P<0.05)。结论超声引导下喉上神经内支阻滞复合静脉全麻用于支撑喉镜下声带息肉摘除术的麻醉效果较好,可维持血流动力学稳定,为术后提供持续稳定的镇痛效果,且不良反应少。Objective To explore the effect of ultrasound-guided internal branch of superior laryngeal nerve block combined with intravenous general anesthesia for vocal cord polyp removal surgery by suspension laryngoscope.Methods A total of 72 patients with vocal cord polyp removal surgery by suspension laryngoscope in our hospital from March 2020 to May 2022 were selected as the research objects and randomly divided into control group and observation group,with 36 cases in each group.The control group was given intravenous general anesthesia,while the observation group was given ultrasound-guided internal branch of superior laryngeal nerve block combined with intravenous general anesthesia.The changes of MAP and HR at different time points of the two groups were observed.The VAS scores at 2h,4h,8h,12h,24h and 48h after operation were compared between the two groups,and the postoperative adverse reactions were analyzed.Results Compared with TO,the MAP and HR of the control group at T1-T5 were significantly increased(P<0.05),while the MAP and HR of the observation group at T1-T5 were not significantly changed(P>0.05).The MAP and HR of the observation group at T1-T5 were significantly lower than those of the control group(P<0.05).The VAS scores of the observation group at 2h,4h,8h,12h,24h and 48h after operation were lower than those of the control group(P<0.05).The incidence of postoperative adverse reactions was 16.67%in the control group,while no adverse reactions were observed in the observation group(P<0.05).Conclusion Ultrasound-guided internal branch of superior laryngeal nerve block combined with intravenous general anesthesia has a good anesthetic effect for vocal cord polyp removal su rgery by suspension laryngoscope,which can maintain hemodynamic stability,provide continuous and stable analgesic effect for postoperative patients,and less adverse reactions.
