CT检查在霉菌性鼻窦炎鉴别诊断中的应用效果及临床价值分析

摘　　要：目的 探讨CT检查在霉菌性鼻窦炎鉴别诊断中的应用效果。方法 遵循随机原则选择2020年1月~2022年4月在我院确诊为霉菌性鼻窦炎的50例患者,均实施CT检查,以手术病理结果为金标准,分析CT检查在霉菌性鼻窦炎诊断中的价值及准确率,分析该方案对病变检出情况。结果 CT检查准确率为96.00%,与金标准相比无显著差异(P>0.05);CT检查发现病变主要累及上颌窦与筛窦,占比80.00%;其次为单侧蝶窦(4.00%)、单侧上颌窦(14.00%)、双侧多鼻窦(2.00%),病变累及范围与金标准对比无统计学意义(P>0.05);50例患者经CT检查发现20.00%存在鼻腔黏膜增厚情况及均匀软组织团块影,但病灶无钙化情况;80.00%可见高密度团块状,并存在钙化灶;发现1例上颌窦内侧壁骨质受损,占比2.00%;发现8例鼻窦骨质硬化与增厚,占比16.00%,其余患者无异常;与金标准结果一致。结论 在霉菌性鼻窦炎鉴别诊断中选择CT检查可通过详细的扫描明确诊断疾病,从而为临床医师制定治疗方案提供参考。Objective To investigate the application of CT in the differential diagnosis of fungal sinusitis.Methods 50 patients with fungal sinusitis diagnosed in our hospital from January 2020 to April 2022 were randomly selected.All patients underwent CT examination.The value and accuracy of CT examination in the diagnosis of fungal sinusitis were analyzed based on the surgical and pathological results.Results The accuracy of CT examination was 96.00%,and there was no significant difference compared with the gold standard(P>0.05);CT showed that the lesions mainly involved maxillary sinus and ethmoid sinus,accounting for 80.00%;The next were unilateral sphenoid sinus(4.00%),unilateral maxillary sinus(14.00%),and bilateral multiple sinuses(2.00%).There was no significant difference between the scope of lesions and the gold standard(P>0.05);CT examination showed that 20.00%of 50 patients had thickening of nasal mucosa and homogeneous soft tissue mass,but no calcification;80.00%showed high density lump and calcification;One case(2.00%)of bone damage in the medial wall of maxillary sinus was found;It was found that 8 cases(16.00%)of bone sclerosis and thickening of nasal sinuses were found,and the other patients had no abnormalities;It is consistent with the surgical results.Conclusion In the differential diagnosis of fungal nasosinusitis,CT can be selected to clearly diagnose the disease through detailed scanning,so as to provide reference for clinicians to formulate treatment plans.
