摘 要:目的采用经心尖人工腱索植入装置建立二尖瓣反流动物模型,并验证装置的稳定性及可靠性。方法以大白猪为实验动物(n=12),在经心表超声引导下,经心尖小戳口植入二尖瓣人工腱索,通过调整人工腱索长度牵拉瓣叶导致二尖瓣反流。在术前、术后即刻、术后1个月、3个月记录心脏超声指标。结果所有模型均建立成功,术中及随访期无死亡,术后即刻二尖瓣均为中度反流。与术前相比,随访3个月时二尖瓣反流程度、心脏各径线均不同程度增加(P<0.05)。结论非体外循环下,超声引导经心尖植入腱索牵拉二尖瓣瓣叶的方法可稳定可靠地建立二尖瓣反流动物模型。Objective To research the procedure for creating an animal model of mitral regurgitation by implanting a device through the apical artificial chordae tendineae,and to assess the stability and dependability of the device.Methods Twelve large white swines were employed in the experiments.Through a tiny hole in the apex of the heart,the artificial chordae tendineae of the mitral valve was inserted under the guidance of transcardiac ultrasonography.Before,immediately after,and one and three months after surgery,cardiac ultrasonography signs were noted.Results All models were successfully established.During the operation and the follow-up,no swines died.Immediately after surgery,the mitral valve experienced moderate regurgitation.Compared with preoperation,there was a variable increase in the amount of regurgitation and the values of heart diameters at a 3-month follow-up(P<0.05).Conclusion In off-pump,the technique of pulling the mitral valve leaflets with chordae tendineae implanted transapically under ultrasound guidance can stably and consistently create an animal model of mitral regurgitation.
