大面积横向推进皮瓣修复颜面侧较大皮肤缺损的研究

摘　　要：目的:探讨应用大面积横向推进皮瓣修复颜面侧较大皮肤缺损的临床效果。方法:2015年1月-2021年12月,笔者医院收治的12例单侧颜面侧较大体表良性肿瘤和瘢痕切除术后患者,颜面侧皮肤缺损平均宽度为5.2 cm。通过创面两侧广泛皮下分离,横向推进,未做皮肤扩张,即完成创面覆盖,从而达到修复的目的。结果:所有患者经6个月~5年随访,切口均愈合良好,无明显瘢痕增生,远期无眼睑外翻等并发症。仅1例患者术后出现切口边缘良性肿物复发,通过再次手术切除后痊愈。结论:应用广泛皮下游离的大面积横向推进皮瓣修复颜面侧较大皮肤缺损,无需皮肤扩张,临床疗效较好,是相对理想的修复方法。Objective To investigate the clinical effect of large-area transverse advancement flap in repairing Large skin defects of the lateral face.Methods From January 2015 to December 2021,12 patients who underwent resection of facial skin benign tumor and scar in the author's hospital were selected,and the average size of the defects was 5.2 cm.To covere the wound,we separated subcutaneous tissue widely on both sides,then advanced the flaps widely without skin expansion.Results All patients were followed up for 6 months to 5 years,and the incisions healed well without obvious scar hyperplasia,and there was no complication such as eyelid ectropion in the long term.Only 1 patient had recurrence of benign tumor at the edge of the incision and was cured by reoperation.Conclusion The extensive subcutaneous free large transverse advancement flap is a relatively ideal method for repairing large skin defects in the lateral facial region.It does not require skin expansion and has good clinical efficacy.
