摘 要:目的观察富血小板血浆联合等速肌力训练对膝骨性关节炎的疗效影响及软骨厚度的变化。方法选取2021年10月—2023年9月甘肃省中医院收治的60例符合研究条件的膝骨关节炎患者,随机分为对照组32例和观察组28例。对照组采用玻璃酸钠注射联合常规肌力训练治疗,观察组采用自体富血小板血浆注射联合等速肌力训练治疗。治疗前及治疗4周、6个月后采用略和麦克马斯特大学骨关节炎指数(WOMAC)对患者进行评估,采用等速肌力测试系统评价患侧膝关节60°/s、90°/s伸肌峰力矩(PT),通过超声诊断仪评估患者关节软骨平均厚度。结果治疗4周以及6个月后两组WOMAC评分、伸膝肌60°/s、90°/s等速测量结果均较治疗前更优,观察组结果优于对照组,差异有统计学意义(P<0.05)。治疗后6个月观察组膝软骨厚度平均值结果优于治疗前以及对照组,差异有统计学意义(P<0.05)。结论富血小板血浆联合等速肌力训练能有效提高膝骨性关节炎患者的膝关节功能、伸膝肌力、保护关节软骨。Objective To observe the therapeutic effect of platelet-rich plasma combined with isokinetic muscle strength training on knee osteoarthritis and the changes in cartilage thickness.Methods Sixty eligible knee osteoarthritis patients admitted to the Gansu Provincial Hospital of Traditional Chinese Medicine from October 2021 to September 2023 were randomly divided into a control group of 32 cases and a observation group of 28 cases.The control group was treated with sodium hyaluronate injection combined with conventional muscle strength training,while the observation group was treated with autologous platelet-rich plasma injection combined with isokinetic muscle strength training.Western Ontarioand McMaster University Osteoarthritis Index(WOMAC)evaluation was performed on patients before treatment and 4 weeks and 6 months after treatment.The peak torque(PT)of the 60°/s and 90°/s extensor muscles on the affected knee joint was evaluated using an isokinetic muscle strength testing system,and the average thickness of the patient's articular cartilage was evaluated using an ultrasound diagnostic instrument.Results After 4 weeks and 6 months of treatment,the WOMAC score and isokinetic measurements of knee extensor muscles at 60°/s and 90°/s in both groups were better than before treatment(P<0.05).Compared between the two groups,the observation group showed better results than the control group(P<0.05).After 6 months of treatment,the average knee cartilage thickness in the observation group was better than before treatment and the control group(P<0.05).Conclusion The combination of platelet-rich plasma and isokinetic muscle strength training can effectively improve knee joint function,extensor muscle strength,and protect joint cartilage in patients with knee osteoarthritis.
