摘　　要：农村家庭结构的日趋小型化冲击着中国传统的家庭养老功能,在这一趋势下研究农村老年人的医疗服务利用行为,有助于明晰新时代下老年人的医疗服务需求。本文利用2015—2018年中国健康与养老追踪调查数据,采用固定效应模型和工具变量法,实证分析了家庭结构变迁对农村老年人医疗服务利用的影响,并探究这一影响在不同的年龄群体、健康状况和经济条件下的异质性。研究发现,相比于家庭结构大型化,家庭结构变迁后的小型化减少了子代对父代的时间支持和经济支持,阻碍了农村老年人医疗服务利用。进一步分析发现,相比于农村低龄老年人,家庭结构小型化对农村高龄老年人医疗服务利用的阻碍作用更强。此外,家庭结构小型化显著减少了健康状况较差和高收入老年人的医疗服务需求。基于此,本文从家庭和政府层面提出了提高农村老年人医疗服务利用的参考路径。The increasing miniaturization of rural family structure is impacting China's traditional family elderly care function.Studying the medical service utilization behavior of elderly people in rural areas under this trend can help clarify the medical service needs of elderly people in the new era.This article uses data from the 2015-2018 China Health and Elderly Care Tracking Survey,using fixed effects models and instrumental variable methods,to empirically analyze the impact of family structure changes on the utilization of medical services by rural elderly people,and explore the heterogeneity of this impact in different age groups,health conditions,and economic conditions.Research has found that compared to the enlargement of family structure,the miniaturization after the transformation of family structure reduces the time and economic support for ffspring to their parents,hindering the utilization of medical services for rural elderly people.Further analysis reveals that compared to rural elderly people,the miniaturization of family structure has a stronger hindering effect on the utilization of medical services by rural elderly people.In addition,the miniaturization of family structure significantly reduces the demand for medical services for elderly people with poor health conditions and high incomes.Based on this,this article proposes a reference path for improving the utilization of medical services for elderly people in rural areas from the perspectives of families and governments.
