疏肝利胆健脾活血方联合熊去氧胆酸治疗原发性胆汁性胆管炎患者的效果及对血清自身抗体水平的影响

摘　　要：目的 分析疏肝利胆健脾活血方联合熊去氧胆酸(UDCA)治疗原发性胆汁性胆管炎(PBC)患者的效果。方法 选取2019年6月至2021年6月我院收治的80例PBC患者为研究对象,按随机数字表法将其分为对照组(n=40,UDCA)和研究组(n=40,疏肝利胆健脾活血方+UDCA)。比较两组的治疗效果。结果 研究组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的透明质酸(HA)、层黏连蛋白(LN)、Ⅲ型前胶原(PC-Ⅲ)及Ⅳ型胶原(Ⅳ-C)水平均降低,且研究组低于对照组(P<0.05)。治疗后,两组的总胆红素(TBIL)、碱性磷酸酶(ALP)、γ-谷氨酰转移酶(GGT)及天门冬氨酸氨基转移酶(AST)水平均降低,且研究组低于对照组(P<0.05)。治疗后,两组的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)及免疫球蛋白M(IgM)水平均降低,且研究组低于对照组(P<0.05)。结论 疏肝利胆健脾活血方联合UDCA治疗PBC患者的疗效确切,不仅能够改善自身抗体水平和肝功能,还能调节肝纤维化指标,临床可进一步推广应用。Objective To analyze the effect of Shugan Lidan Jianpi Huoxue prescription combined with ursodeoxycholic acid(UDCA)in the treatment of patients with primary biliary cholangitis(PBC).Methods A total of 80 patients with PBC admitted in our hospital from June 2019 to June 2021 were selected as the research objects and divided into control group(n=40,UDCA)and study group(n=40,Shugan Lidan Jianpi Huoxue prescription+UDCA)according to the random number table method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the levels of hyaluronic acid(HA),laminin(LN),procollagen typeⅢ(PC-Ⅲ)and typeⅣcollagen(Ⅳ-C)in the two groups decreased,and those in the study group were lower than the control group(P<0.05).After treatment,the levels of total bilirubin(TBIL),alkaline phosphatase(ALP),gamma-glutamyltransferase(GGT)and aspartate aminotransferase(AST)in the two groups decreased,and those in the study group were lower than the control group(P<0.05).After treatment,the levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in the two groups decreased,and those in the study group were lower than the control group(P<0.05).Conclusion The Shugan Lidan Jianpi Huoxue prescription combined with UDCA in the treatment of PBC patients has definite curative effect,it can not only improve the level of autoantibodies and liver function,but also regulate the indexes of liver fibrosis,which can be further promoted and applied in clinical practice.
