摘 要:目的:探究动态增强磁共振成像(DCE-MRI)定量参数鉴别诊断肺部肿瘤良恶性的价值。方法:选取中国人民解放军陆军第七十三集团军医院2022年2月—2023年4月收治的100例肺部肿瘤患者,所有患者入院后均接受常规磁共振成像(MRI)平扫、DCE-MRI扫描及病理检查。比较良恶性肺部肿瘤DCE-MRI定量渗透性参数[血液回流常数(K_(ep))、容积转运常数(K^(trans))、血浆容积分数(V_(p))、血管外细胞外间隙容积分数(V_(e))],分析DCE-MRI定量参数鉴别诊断肺部肿瘤良恶性的价值。结果:100例肺部肿瘤患者中,经病理检查确诊为41例良性肿瘤,59例恶性肿瘤。恶性肿瘤组的K^(trans)、K_(ep)均比良性肿瘤组高(P<0.05)。两组V_(e)、V_(p)比较,差异均无统计学意义(P>0.05)。绘制受试者操作特征(ROC)曲线,结果显示,K^(trans)、K_(ep)单独及联合诊断肺部肿瘤良恶性的曲线下面积(AUC)为0.835、0.872、0.945,均有一定诊断价值。当K^(trans)、K_(ep)分别为0.525 min^(-1)、0.920 min^(-1)时,可获得最佳诊断效能,且二者联合诊断的价值较高。结论:DCE-MRI定量参数鉴别诊断肺部肿瘤良恶性的价值较高,可通过检测其K^(trans)、K_(ep)值来判断肺部肿瘤的良恶性。Objective:To explore the value of quantitative parameters of Dynamic Enhanced Magnetic Resonance Imaging(DCE-MRI)for differential diagnosis of benign and malignant lung tumors.Method:A total of 100 patients with lung tumor were selected from 73rd Group Military Hospital of the Chinese People's Liberation Army from February 2022 to April 2023,all patients received routine magnetic resonance imaging(MRI)plain scan,DCE-MRI scan and pathological examination after admission.The DCE-MRI quantitative permeability parameters[blood reflux constant(K_(ep)),volume transfer constant(K^(trans)),plasma volume fraction(V_(p)),extracellular space volume fraction(V_(e))]of benign and malignant lung tumors were compared,the value of DCE-MRI quantitative parameters for the differential diagnosis of benign and malignant lung tumors were analyzed.Result:Among 100 patients with lung tumour,41 benign tumour and 59 malignant tumour were diagnosed by pathological examination.The K^(trans) and K_(ep) of the malignant tumour group were higher than those of the benign tumour group(P<0.05).There were no significant differences in V_(e) and V_(p) between malignant tumor group and benign tumor group(P>0.05).Receiver operating characteristic(ROC)curve was drawn,and the results showed that the area under the curve(AUC)of K^(trans) and K_(ep) alone and combined in the diagnosis of benign and malignant lung tumors were 0.835,0.872 and 0.945,all of which had certain diagnostic value.When K^(trans) and K_(ep) were 0.525 min^(-1) and 0.920 min^(-1),respectively,the best diagnostic efficiency can be obtained,and the combined diagnosis value of the two was higher.Conclusion:DCEMRI quantitative parameters have high value in differential diagnosis of benign and malignant lung tumors.K^(trans) and K_(ep) values can be used to determine benign and malignant lung tumors.
