摘　　要：目的 分析不同术式治疗GartlandⅢ型不稳定性肱骨髁上骨折患儿的效果。方法 选取本院2020年1月~2023年1月收治的GartlandⅢ型不稳定性肱骨髁上骨折患儿80例。随机分为观察组40例(采用闭合复位外侧两枚克氏针固定术)和对照组40例(采用切开复位双侧交叉克氏针内固定术)。比较两组手术效果、临床相关指标以及并发症情况。结果 两组优良率比较差异无统计学意义(P>0.05);观察组手术时间、术中出血量以及住院时间低于对照组,并发症发生率高于对照组(P<0.05);骨折愈合时间:观察组与对照组比较差异不显著(P>0.05)。结论 两组术式对效果差异不大,但观察组并发症较多,而对照组手术时间、住院时间较长且术中出血较多,两种术式对GartlandⅢ型不稳定性肱骨髁上骨折患儿治疗均有优劣,可根据患儿状况进行选择。Objective:To analyze the effects of different surgical methods in the treatment of children with Gartland type III unstable supracondylar humeral fractures.Methods:Eighty children with Gartland type III unstable supracondylar humeral fractures who were admitted to the hospital from January 2020 to January 2023 were selected,and divided into the observation group(40 children treated with closed reduction and lateral fixation with two Kirschner wires),and the control group(40 children treated with open reduction and bilateral internal fixation with crossed Kirschner wires).Surgical outcomes,related clinical indicators,and complications were compared between the two groups.Results:There was no statistically significant difference in the excellent and good rate between the groups(P>0.05).Surgical time,intraoperative blood loss,and hospital stay of the observation group were significantly shorter/less than those of the control group.The incidence of complications in the observation group was significantly higher than that in the control group(P<0.05).The fracture healing time of the two groups was similar(P>0.05).Conclusion:Closed reduction and lateral fixation with two Kirschner wires can achieve a similar effect to open reduction and bilateral internal fixation with crossed Kirschner wires.However,the former has more complications.The duration of the latter and the length of hospital stay after the latter surgery is longer,and blood loss during the latter is greater.Therefore,the surgery should be selected based on the patient's condition.
