基于数据挖掘探讨国医大师雷忠义治疗冠心病PCI术后病人的用药规律

摘　　要：目的:采用数据挖掘方法分析总结国医大师雷忠义治疗冠心病经皮冠状动脉介入治疗(PCI)术后的用药规律。方法:收集整理雷忠义教授2014年1月—2022年6月治疗冠心病PCI术后的处方用药,借助Excel 2019、IBM SPSS Statistics 25.0、SPSS Modeler 18.0、Cytoscape 3.9.1等软件对纳入处方用药进行频数统计、药物性味归经分析、关联规则分析、聚类分析、复杂网络分析。结果:筛选后共纳入处方61个,其中36个处方涉及中成药,所纳入处方共涉及药物148味,排名居前10位的高频药物依次为丹参、五味子、三七、麦冬、陈皮、黄芪、瓜蒌、薤白、黄连、人参;药物四气以温、寒、平为主,五味以苦、甘、辛为主,归经以肺、肝、心、脾为主;关联规则发现常用的对药为五味子-麦冬、丹参-三七,常用的角药为五味子-陈皮-麦冬、丹参-陈皮-麦冬;聚类分析得到4个聚类组合,复杂网络发现核心方为雷忠义自拟方养心活血汤。结论:国医大师雷忠义治疗冠心病PCI术后从“痰瘀”立论,注重祛邪与扶正兼顾,善用自拟方养心活血汤,动态辨证,随证加减。Objective:To analyze and summarize the medication rules of Chinese medical master Lei Zhongyi for the treatment of coronary heart disease after percutaneous coronary intervention(PCI)based on data mining.Methods:The prescription medications of Chinese Medical Master Lei Zhongyi for treatment of coronary artery disease after PCI from January 2014 to June 2022 were collected and organized.Excel 2019,IBM SPSS Statistics 25.0,SPSS Modeler 18.0,Cytoscape3.9.1 and other software were used to perform frequency statistics,drug flavor classification analysis,association rule analysis,cluster analysis,and complex network analysis for included prescription drugs.Results:After screening,there were 61 prescriptions included,36 involved Chinese patent medicines,and a total of 128 flavors were included.The top 10 high-frequency drugs were salvia miltiorrhiza,schisandra,Notoginseng,ophiopogon,Tangerine peel,Astragalus,Gualou,Allium Macrostemon,Coptis,ginseng.The four qi of medicine were mainly warm,cold and flat,the five flavors were mainly bitter,sweet and xin,and the channel entry were mainly lung,liver,heart and spleen.The association rules showed that the commonly drug combinations were Schisandra chinensis-maitake,danshen-panax ginseng,and the commonly used angiosperms were schisandra chinensis-chenp-i maitake,danshen-chenp-i maitake.The cluster analysis showed there were 4 cluster combinations,and the complex network found that the core formula was Yangxin Huoxue Decoction by Lei Zhongyi.Conclusion:Chinese Medical Master Lei Zhongyi treated coronary heart disease after PCI from the"phlegm stasis",focusing on the elimination of evil and support both positive,with self-formulated prescription Yangxin Huoxue Decoction,dynamic identification,add and subtract according to the evidence.
