健脾温肾法论治2型糖尿病探源

摘　　要：糖尿病在中医学属“消渴”范畴,历代文献中都有脾肾与消渴相关的论述。中医学关于脾的概念既包含“脾”自身的生理功能,又涵盖现代医学所指的“胰”的生理功能。基于“脾胰同源”理论,结合现代医学对胰腺的认识,分析探究脾肾阳虚与消渴发生的关系。胰的内分泌功能属脾,外分泌功能属肾,2型糖尿病是胰腺功能障碍引起的津液代谢失常。以我国现阶段的国情和群众体质变化为依据,提出脾肾阳虚是当前2型糖尿病的常见证型,除常用的养阴清热等治法外,健脾温肾法也是2型糖尿病重要的治疗方法之一。Diabetes mellitus belongs to the category of“xiaoke”in traditional Chinese medicine.There are discussions about the relationship between spleen and kidney and xiaoke in ancient literature.The concept of spleen in traditional Chinese medicine not only includes the physiological function of spleen itself,but also covers the physiological function of pancreas in modern medicine.Based on the theory of“spleen and pancreas sharing the same origin”and the understanding of pancreas in modern medicine,the relationship between Yang deficiency of spleen and kidney and the occurrence of xiaoke is analyzed.The endocrine function of the pancreas belongs to the spleen,and the exocrine function belongs to the kidney.Type 2 diabetes mellitus is a disorder of body fluids metabolism caused by pancreatic dysfunction.Based on the current situation of our country and the changes of people′s constitution,it is proposed that the Yang deficiency of spleen and kidney is a common syndrome of type 2 diabetes mellitus.In addition to the commonly used treatments such as nourishing Yin and clearing heat,strengthening spleen and warming kidney is also one of the important treatment methods for type 2 diabetes mellitus.
