颊针对于原发性痛经的镇痛疗效及对血清PGF_(2α)和PGE_(2)水平变化的影响

摘　　要：目的:探讨颊针对原发性痛经的镇痛疗效及对血清PGF_(2α)和PGE_(2)水平变化的影响。方法:将60例诊断为原发性痛经患者按随机数字表法分为颊针组和对照组,每组30例。颊针组取中焦穴、下焦穴、腰穴和骶穴,1次/d,每次30 min;对照组口服布洛芬缓释胶囊,2次/d,每次300 mg。两组均于月经前3 d开始,至月经期第3天,连续治疗3个疗程,每个疗程为1个月经周期,比较两组疼痛视觉模拟(VAS)评分、COX痛经症状评价量表(CMSS)评分,检测两组血清前列素F_(2α)(PGF_(2α))和血清前列素E_(2)(PGE_(2))的水平变化。结果:两组治疗后的VAS评分、CMSS评分以及血清PGF_(2α)和PGE_(2)均较治疗前显著降低,治疗后颊针组较对照组的VAS评分、CMSS评分以及血清PGF_(2α)和PGE_(2)均显著降低,差异具有统计学意义(P<0.05)。结论:颊针较布洛芬缓释胶囊对原发性痛经镇痛效果更佳,并可缓解其相应伴随症状,镇痛机制可能与PGF_(2α)和PGE_(2)水平变化有关。Objective:To explore the analgesic effect of buccal acupuncture in treatment of primary dysmenorrhea and its influence to serum levels of PGF_(2α) and PGE_(2).Methods:60 patients diagnosed with primary dysmenorrhea were randomly divided into the buccal acupuncture group and the control group,with 30 cases in each group.The buccal acupuncture group was treated with needling middle-jiao point,lower-jiao point,waist point,and sacral point,30 minutes each time;the control group was treated with Ibuprofen Sustained-Release Capsule(300 mg),twice a day.Both groups started treatment 3 days before menstruation and continued until 3rd day of period,and the treatment was 3 consecutive courses.Scores of VAS and CMSS,as well as serum levels of PGF_(2α) and PGE_(2) were detected in the two groups.Results:The scores of VAS and CMSS,as well as the serum levels of PGF_(2α) and PGE_(2) were significantly reduced after the treatment compared with those before the treatment in the two groups,and the reductions were more significant in the buccal acupuncture group than those in the control group(P<0.05).Conclusion:Buccal acupuncture has a better analgesic effect than Ibuprofen Sustained-release Capsule in treatment of primary dysmenorrhea,it can alleviate the accompanying symptoms.The analgesic mechanism may be related to the level changes of PGF_(2α) and PGE_(2).
