摘　　要：总结郭长青教授治疗扳机指的学术思想。郭教授认为本病的发生由邪结经筋、关节错缝与隐性粘连等因素所致,治疗需以针刀松解“筋”的粘连,手法纠正“骨”的错缝,穴位点刺祛除“脉”的瘀滞,功能锻炼改善“肉”的萎缩。本研究阐述了超声检查在扳机指治疗中的意义,针具的选用依据和操作方法,针刀“线型松解”的优势与不足,穴位应用的目的和方法,手法治疗的原则和方式及康复指导中的注意事项。指出了在治疗过程中如何避免神经血管损伤、针刀过度切割、术后粘连或发生感染和肌腱断裂等问题的方法。本研究讲解了对软组织“隐性粘连”的处理方法,对是否需“刺至骨”进行了说明。This study summarized Professor GUO Changqing's academic thoughts on treatment of trigger finger.Professor GUO believes that the occurrence of this disease is caused by meridian pathological phenomenon,slight dislocation of the joint,recessive adhesion and other factors.It is necessary to release compression of ‘tendon' with acupotomy,correct dislocation of ‘joint' by means of manipulation,prick acupoint to remove stasis in the ‘channel' and relieve atrophy of ‘muscle' through functional exercise.This study expounded the significance of ultrasonic examination in treatment of trigger finger,the selection and manipulation of acupotomy,the advantages and disadvantages of the ‘linear release' of acupotomy,the purposes and methods of acupoints' application,the principles and methods of manipulative therapy and the matters needing attention in the rehabilitation guidance.It also pointed out the methods of avoiding neurovascular injury,excessive acupotomy cutting,postoperative adhesion and infection,as well as tendon rupture.It explained the treatment method of ‘recessive adhesion' and explained the need of ‘puncturing to bone'.
