李乃庚教授运用“护阳益阴”理论治疗儿科疾病验案4则

摘　　要：小儿为稚阴稚阳之体,脏腑娇嫩,临床中常因“祛邪太过”“形寒饮冷”“恣食肥甘厚味”“先天禀赋不足”“失治误治”“久病传变”“滥用抗生素、激素”等导致气阴不足。李乃庚教授认为此类患儿单依“用药立足祛邪”不能取效,如祛邪太过更易伤正,应运用“护阳益阴”理论进行治疗。“护阳益阴”理论的本质即调其偏颇以更好地扶正祛邪。李教授鉴于阴阳互根之理,阴虚及阳,腠理难密,故予玉屏风散加味益气养阴、固表敛汗,联合止汗散外用敛阴止汗、潜阳固涩治疗小儿汗证。小儿气血不足,中阳不振致腹痛绵绵,常予黄芪建中汤加减以温脾建中、补气益血。李教授认为小儿过敏性紫癜病久易由实转虚,则致气阴不足,久病及肾则成脾肾两虚、浮阳上越之证,治宜益气温肾、潜阳入阴,方选右归丸加减。小儿久咳易耗气伤阴,治宜益气养阴、化痰止咳,常予验方黄芪止嗽饮加减,方中重用黄芪,疗效显著。Children's constitutions are characterized by young yin and young yang,and their internal organs are delicate.In clinical practice,patients'insufficiency of qi and yin is often caused by“excessive expulsion of the evils”,“taking cold food or beverages after being exposed to coldness”,“over-consumption of fatty,sugary,starchy,and spicy foods”,“born with poor health”,“treatment failure or misuse”,“long-standing illnesses”,and misuse of antibiotics and hormones,and so on.Professor LI Naigeng believes that such children cannot achieve therapeutic effects solely by relying on“medication based on dispelling the evils”.Moreover,if the dispelling of the evils is too much,it is more likely to harm the patients'constitution.Therefore,the theory of“protecting yang and benefiting yin”should be applied for treatment.The essence of the theory is to adjust its bias in order to better support the positive and eliminate the evil.Professor Li is good at using the theory of“protecting yang and benefiting yin”to treat pediatric diseases.According to this theory,prescriptions and drugs have achieved good clinical effects.This paper mainly introduces four cases of professor LI naigeng's application of the theoryin the treatment of children's“sweating disease,abdominal pain,purpura,cough and other diseases.Professor LI,in view of the theory of yin and yang mutual root,yin deficiency lead to yang deficiency,the coupling is difficult to be dense,so modified Yupingfeng San(玉屏风散)is used to benefit the qi and nourish the yin,consolidate the surface convergence of sweating combined with the anti-perspiratory external convergence of yin to stop sweating,submerged in the yang and fixed astringent treatment of paediatric sweating disease.For pediatric abdominal pain caused by insufficient qi and blood,professor LI often gave Huangqi Jianzhong Tang(黄芪建中汤)to warm the spleen,replenish qi and benefit blood.Professor LI believed that pediatric allergic purpura can be easily converted from solid
