摘　　要：目的:探讨经济毒性在晚期肺癌病人症状负担与失志综合征中的中介作用。方法:采用便利抽样法选取2022年2月-2023年2月在郑州市某三级甲等医院住院的292例晚期肺癌病人作为研究对象,采用一般资料调查表、癌症病人报告结局的经济毒性量表、安德森症状量表、简化版失志量表进行问卷调查。应用SPSS 26.0软件及AMOS 28.0软件进行统计分析。结果:晚期肺癌病人经济毒性总分为(11.66±4.35)分,症状严重程度总分为(123.14±14.23)分,症状困扰程度总分为(37.96±6.31)分,失志综合征总分为(18.73±5.11)分;Pearson相关分析显示,经济毒性总分与症状严重程度、症状困扰程度及失志综合征得分均呈负相关(r值分别为-0.601,-0.537,-0.578,P<0.01),失志综合征得分与症状严重程度及症状困扰程度得分均呈正相关(r值分别为0.502,0.538,P<0.01)。Bootstrap法显示,经济毒性在症状负担与失志综合征间起部分中介作用,经济毒性在症状严重程度与失志综合征间的间接效应占总效应的33.15%。经济毒性在症状困扰程度与失志综合征间的间接效应占总效应的35.26%。结论:晚期肺癌病人经济毒性在症状负担与失志综合征间起部分中介作用,医护人员应为病人提供全面支持,降低其经济毒性,进而降低其失志水平。Objective:To investigate the mediating role of economic toxicity in symptom burden and demoralization syndrome in advanced lung cancer patients.Methods:A total of 292 patients with advanced lung cancer admitted to a grade-A hospital in Zhengzhou City from February 2022 to February 2023 were selected by convenience sampling method.General data questionnaire,Comprehensive Scores for Financial Toxicity based on the Patient-Reported Outcome Measures,the M.D.Anderson Symptom Inventory and the Short Demoralization Scale were used to investigate.SPSS 26.0 software and AMOS 28.0 software were used for statistical analysis.Results:The total score of economic toxicity,symptom severity and symptom distress of advanced lung cancer patients were(11.66±4.35),(123.14±14.23)and(37.96±6.31)respectively.The total score of demoralization syndrome was(18.73±5.11).Pearson correlation analysis showed that the total score of economic toxicity was negatively correlated with symptom severity,symptom distress and demoralization syndrome(r=-0.601,-0.537,-0.578,P<0.01).Demoralization syndrome was positively correlated with symptom severity and symptom distress(r=0.502,0.538,P<0.01).Bootstrap method showed that economic toxicity played a partial mediating role between symptom burden and demoralization syndrome,and the indirect effect value of economic toxicity between symptom severity and demoralization syndrome was 33.15%.The indirect effect value of economic toxicity between symptom distress and demoralization syndrome was 35.26%.Conclusions:Economic toxicity plays a partial mediating role between symptom burden and demoralization syndrome in patients with advanced lung cancer.Medical staff should provide comprehensive support for patients to reduce their economic toxicity and thus reduce the level of demoralization.
