摘　　要：老年心力衰竭病人病情易反复,症状负担重,且猝死风险高,往往面临与个人意愿不符的医疗决策,因此对其实施预立医疗照护计划尤为重要。该文综述了预立医疗照护计划相关概念、对老年慢性心力衰竭病人应用预立医疗照护计划的必要性、预立医疗照护计划在老年慢性心力衰竭病人中应用的形式及效果、对老年慢性心力衰竭病人实施预立医疗照护计划存在的挑战及建议,以期为我国老年慢性心力衰竭病人开展预立医疗照护计划及安宁疗护提供参考。Elderly patients with heart failure are prone to relapse,have a heavy burden of symptoms,a high risk of sudden death,and often faced with medical decisions that were inconsistent with their personal wishes,so it is particularly important to implement advance care planning for them.This paper reviewed the concepts related to advance care planning,the necessity of applying advance care planning to elderly patients with chronic heart failure,the forms and effects of advance care planning in elderly patients with chronic heart failure,the challenges and suggestions of implementing advance care planning for elderly patients with chronic heart failure,in order to provide reference for the development of advance care planning and hospice care of elderly patients with chronic heart failure in China.
