基于数据挖掘探究石氏伤科运用芳香类中药治疗腰椎间盘突出症的用药规律

摘　　要：通过收集并筛选2018年1月1日至2023年7月31日上海中医药大学附属龙华医院骨伤科门诊治疗腰椎间盘突出症的中药处方,运用R语言对筛选出的处方进行用药频次分析、关联规则分析、聚类分析、相关性分析,探究了石氏伤科运用芳香类中药治疗腰椎间盘突出症的用药规律和特点。结果共筛选得到了2811处方,使用频次>100的芳香中药45味,功效以芳香补益、芳香活血、芳香除湿为主,性味偏辛、温、苦,归经集中在肝经、脾经、肺经、肾经,关联规则得到了以当归、川芎、川牛膝为核心的芳香药物组合,聚类分析得到了以圣愈汤合身痛逐瘀汤为主的核心处方,相关性分析得到了广藿香-佩兰的药对关联性最强。结果表明芳香类中药在腰椎间盘突出症的治疗中应用广泛,体现了石氏伤科调和气血的治疗原则,为中医药精准施治提供借鉴和参考。Through collecting and screening the traditional Chinese medicine prescriptions for the treatment of lumbar disc herniation in the Orthopedics and Traumatology Clinic of Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 1,2018 to July 31,2023,and the screened prescriptions were subjected to the analysis of medication frequency,association rules,clustering and correlation by using the R language.The medication pattern and characteristics of Shi’s traumatology using aromatic traditional Chinese medicine to treat lumbar disc herniation based on data mining were explored.A total of 2811 prescriptions were obtained,45 aromatic traditional Chinese medicines with a frequency more than 100 in these prescriptions.The efficacy was mainly based on aromatic tonic,aromatic blood circulation activation and aromatic dehumidification,and most drug properties were pungent,warm and bitter,and their attributed meridians were concentrated in the liver meridian,spleen meridian,lung meridian and kidney meridian.The correlation rule obtained the combination of aromatic traditional Chinese medicines centered on Angelicae sinensis radix,Chuanxiong rhizoma and Cyathulae radix,and the clustering analysis obtained the core prescription centered on the Shengyu Decoction combined with the Shentong Zhuyu Decoction,and the correlation analysis obtained the strongest pairwise correlation between Pogostemonis herba and Eupatorii herba.The results indicate that aromatic traditional Chinese medicines are widely used in the treatment of lumbar disc herniation,reflecting the therapeutic principle of regulating qi and blood of Shi’s traumatology,and providing reference for the precise administration of traditional Chinese medicine.
