摘 要:目的:测定头孢曲松/他唑巴坦对产超广谱β-内酰胺酶大肠埃希菌(extended-spectrumβ-lactamase producing Escherichia coli,ESBLs-Eco)的最低抑菌浓度(minimum inhibitory concentration,MIC)、抗生素后效应(post antibiotic effect,PAE)和β-内酰胺酶抑制剂后效应(postβ-lactamase inhibitor effect,PLIE)。方法:采用微量肉汤稀释法测定其MIC。采用菌落计数法测定菌落形成单位,以培养时间为横坐标,菌落数的对数值为纵坐标绘制细菌生长曲线,并计算PAE和PLIE。结果:与头孢曲松单方相比,头孢曲松/他唑巴坦对ESBLs-Eco的MIC明显降低;在较低药物浓度下,头孢曲松/他唑巴坦对ESBLs-Eco的PAE和PLIE为负值或较短。当后效应为正值时,其PLIE>PAE。头孢曲松/他唑巴坦对ESBLs-Eco的PAE和PLIE随着药物浓度的增加而延长,当药物浓度达到800×MIC时,PAE为3.65 h,PLIE>12.86 h。结论:头孢曲松/他唑巴坦对ESBLsEco的PAE和PLIE具有浓度依赖效应。PLIE和PAE在维持头孢曲松/他唑巴坦于体内发挥持久的抗菌活性中起到了重要作用,且PLIE起到的作用强于PAE。OBJECTIVE To determine the minimal inhibitory concentration(MIC),post-antibiotic effect(PAE),and postβ-lactamase inhibitor effect(PLIE)of ceftriaxone/tazobactam against extended-spectrumβ-lactamase-producing Escherichia coli(ESBLs-Eco).METHODS The MIC was determined by the broth microdilution method.The colony forming unit was determined by colony counting method.The bacterial growth curve was drawn with culture time as Abscissa and logarithm of colony number as ordinate,and PAE and PLIE were calculated.RESULTS Compared with ceftriaxone alone,the MIC of ceftriaxone/tazobactam to ESBLs-Eco was significantly lower,and the PAE and PLIE of ceftriaxone/tazobactam to ESBLs-Eco were negative or shorter at lower drug concentration.When the after-effect is positive,it's PLIE>PAE.The PAE and PLIE of ceftriaxone/tazobactam to ESBLs-Eco were prolonged with the increase in drug concentration.When the concentration of ceftriaxone/tazobactam reached 800×MIC,PAE was 3.65 h and PAE>12.86 h.CONCLUSION Ceftriaxone/tazobactam has a concentration-dependent effect on PAE and PLIE of ESBLs-Eco.PLIE and PAE play an important role in maintaining the lasting antibacterial activity of ceftriaxone/tazobactam in vivo,and the role of PLIE is stronger than that of PAE.
