紫榆汤联合锡类散保留灌肠治疗热毒炽盛证溃疡性直肠炎的疗效

摘　　要：目的:探讨紫榆汤联合锡类散保留灌肠治疗热毒炽盛证溃疡性直肠炎的临床疗效。方法:选择2021年3月至2023年3月于我院就诊的78例热毒炽盛证溃疡性直肠炎患者,随机分为观察组和对照组,各39例,观察组予紫榆汤+锡类散保留灌肠,对照组予锡类散保留灌肠,每天1次,连续治疗1个月。比较2组治疗总有效率,治疗前后黏液脓血便、肛门坠胀感、频繁便意感、肠镜检查情况评分及血清白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平。结果:观察组治疗总有效率(92.3%,36/39)明显高于对照组(74.4%,29/39),P<0.05。治疗后,2组患者黏液脓血便、肛门坠胀感、频繁便意感、肠镜检查情况评分及血清IL-6、hs-CRP、TNF-α水平均明显下降,其中观察组各症状评分、肠镜检查情况评分及各炎症因子水平更低,P<0.05。结论:与锡类散保留灌肠相比,联合应用紫榆汤保留灌肠治疗热毒炽盛证溃疡性直肠炎疗效更好,可更有效地缓解患者症状、降低炎症因子水平。Objective To explore the clinical efficacy of retention enema of Ziyu Decoction combined with Xilei Powder in treatment of heat-toxin exuberance pattern of ulcerative proctitis.Methods A total of 78 patients with heat-toxin exuberance pattern of ulcerative proctitis who were treated in our hospital from March 2021to March 2023were randomly divided into the observation group and the control group,with 39cases in each group.The observation group was given retention enema of Ziyu Decoction and Xilei Powder,while the control group was given retention enema of Xilei Powder,once a day for continuous treatment for 1month.The total effective rate of treatment,bloody mucopurulent stool,anal collapse and distention,frequent bowel movements,scores of colonoscopy before and after treatment,and serum levels of interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)were compared between the two groups.Results The total effective rate of the observation group(92.3%,36/39)was significantly higher than that of the control group(74.4%,29/39)(P<0.05).After treatment,bloody mucopurulent stool,anal collapse and distention,frequent bowel movements,scores of colonoscopy and serum levels of IL-6,hs-CRP,TNF-αin the two groups were significantly decreased,and the scores of symptoms,scores of colonoscopy and levels of inflammatory factors in the observation group were decreased(P<0.05).Conclusion Compared with retention enema of Xilei Powder,retention enema of combined Ziyu Decoction with Xilei Powder in the treatment of heat-toxin exuberance pattern of ulcerative proctitis has better efficacy,which can more effectively relieve the symptoms of patients and reduce the levels of inflammatory factors.
