腹腔镜辅助经肛改良Soave术治疗先天性巨结肠的疗效分析

摘　　要：目的:探讨腹腔镜辅助经肛改良Soave术治疗先天性巨结肠的疗效。方法:回顾2020年1月至2023年3月我院收治的62例先天性巨结肠患儿资料,按不同手术治疗方式分为腹腔镜组(腹腔镜辅助经肛改良Soave术,32例)和常规组(常规经肛改良Soave术,30例),比较2组围术期相关指标、术后并发症及术后6个月肛门功能。结果:62例患儿均顺利完成手术。腹腔镜组手术时间、术后下床活动时间、住院时间均短于常规组,术中出血量少于常规组,P<0.05。腹腔镜组术后并发症发生率低于常规组,患儿术后6个月肛门功能优于常规组,P<0.05。结论:腹腔镜辅助经肛改良Soave术治疗先天性巨结肠效果优于常规经肛改良Soave术,不仅可缩短手术时间、下床活动及住院时间,减少术中出血量,降低术后并发症发生率,而且可提高肛门功能状况。Objective To investigate the efficacy of laparoscopic-assisted transanal modified Soave surgery for Hirschsprung's disease.Methods The clinical data of 62children with Hirschsprung's disease admitted to our hospital from January 2020to March 2023were retrospectively analyzed.According to different surgical treatment methods,they were divided into the laparoscopic group(laparoscopic-assisted transanal modified Soave surgery,32cases)and the conventional group(conventional transanal modified Soave surgery,30cases).The perioperative related indicators,postoperative complications and anal function 6months after operation were compared between the two groups.Results Total 62children underwent surgery successfully.The operation time,postoperative ambulation time and hospital stay in the laparoscopic group were shorter than those in the conventional group,and the intraoperative blood loss was less than that in the conventional group(P<0.05).The incidence of postoperative complications in the laparoscopic group was lower than that in the conventional group,and the anal function 6months after operation was better than that in the conventional group(P<0.05).Conclusion The effect of laparoscopic-assisted transanal modified Soave surgery for Hirschsprung's disease is better than that of transanal modified Soave surgery.It can not only shorten the operation time,postoperative ambulation time and hospital stay,reduce the intraoperative blood loss,reduce the incidence of postoperative complications,but also improve anal function.
