儿童鼻咽侧位片测量参数对腺样体肥大诊断的价值分析

摘　　要：目的 分析儿童鼻咽侧位片测量参数对腺样体肥大诊断的价值。方法 选取2022年10月至2023年10月河南省儿童医院收治的76例腺样体肥大患儿作为病例组研究对象,另外将76例健康儿童纳入作为对照组,纳入研究者均接受鼻咽侧位片检查,对比两组鼻咽侧位片检查参数,包括腺样体厚度与鼻咽腔气道宽度比值(A/N)、鼻咽自由通气道与气管通气道的比值(NS/TS)、下颌升支高度与总前面部高度的比值(Ar-Go/N-Me)、后鼻棘-咽顶点距(PNS-R)、后鼻棘-上咽壁距(PNS-UPW)、软腭后-软腭后咽壁距(SPP-SPPW)、上中切牙长轴与下中切牙长轴的交角(∠UI-LI)、上中切牙长轴和上牙槽座点连线的交角(∠UI-NA)、下中切牙长轴和下牙槽座点连线的交角(∠LI-NB)。将鼻内镜结果做为金标准,计算鼻咽侧位片诊断效能。结果 病例组A/N、NS/TS、Ar-Go/N-Me、PNS-R、PNS-UPW、SPPSPPW、∠UI-LI、∠UI-NA、∠LI-NB检测值均高于对照组,差异有统计学意义(P <0.05);鼻内镜结果阳性确诊数、阴性确诊数分别为76例、0例,鼻咽侧位片阳性例数、阴性例数分别为75例、1例,鼻咽侧位片诊断准确度、灵敏度均为98.68%。结论 儿童鼻咽侧位片测量参数(A/N、NS/TS、Ar-Go/N-Me、PNS-R、PNS-UPW、SPP-SPPW、∠UILI、∠UI-NA、∠LI-NB)可作为儿童腺样体肥大的辅助诊断依据。Objective To analyze the value of lateral nasopharyngeal radiograph measurement parameters in the diagnosis of adenoid hypertrophy in children.Methods 76 children with adenoid hypertrophy admitted to Henan Children's Hospital from October 2022 to October 2023 were selected as the case group,and another 76 healthy children were included as the control group.All participants underwent lateral nasopharyngeal radiography.The parameters of the lateral nasopharyngeal radiographs of the two groups were compared,including the ratio of adenoid thickness to nasopharyngeal airway width(A/N),the ratio of free nasopharyngeal airway to tracheal airway(NS/TS),the ratio of mandibular ascending height to total anterior facial height(Ar-Go/N-Me),the distance from the posterior nasal spine to the pharyngeal apex(PNS-R),the distance from the posterior nasal spine to the upper pharyngeal wall(PNS-UPW),the distance from the posterior soft palate to the posterior pharyngeal wall(SPPSPPW),the angle between the long axis of the upper central incisor and the long axis of the lower central incisor( ∠ UI-LI),the angle between the long axis of the upper central incisor and the line connecting the upper alveolar point( ∠ UI-NA),and the angle between the long axis of the lower central incisor and the line connecting the lower alveolar point( ∠ LI-NB).The results of nasal endoscopy were used as the gold standard to calculate the diagnostic efficacy of lateral nasopharyngeal radiographs.Results The detection values of A/N,NS/TS,Ar-Go/N-Me,PNS-R,PNS-UPW,SPP-SPPW,∠ UI-LI,∠ UI-NA,∠ LI-NB in the case group were all higher than those in the control group,and the differences were statistically significant(P<0.05);the number of positive and negative diagnoses by nasal endoscopy were 76 and 0,respectively,and the number of positive and negative cases by lateral nasopharyngeal radiographs were 75 and 1,respectively,with the diagnostic accuracy and sensitivity of lateral nasopharyngeal radiographs both being 98.68%.Conclusion The measurement p
