摘 要:目的 分析脊柱微创手术对骨质疏松性椎体骨折的影响。方法 选取2020年1月—2022年12月江南大学附属医院收治的80例骨质疏松性椎体骨折患者作为研究对象,按照单双号分组,将其分为开放组(n=35)和微创组(n=45),开放组行传统开放手术,微创组行脊柱微创手术。比较两组的围术期指标、椎体状态、愈合情况、并发症发生情况。结果 术后,微创组的手术、出院时间均短于开放组,差异有统计学意义(P均<0.05)。微创组的伤椎前缘高度、伤椎中部高度均高于开放组,Cobb角低于开放组,差异有统计学意义(P均<0.05)。微创组的日本骨科协会评分、日常生活活动评分均高于开放组,视觉模拟评分低于开放组,差异有统计学意义(P均<0.05)。微创组的并发症发生率(0)低于开放组(11.43%),差异有统计学意义(χ^(2)=5.414,P<0.05)。结论 脊柱微创手术的并发症少,能快速恢复伤椎高度,下调Cobb角,减轻椎体疼痛感,恢复脊柱功能,推动骨质疏松性椎体骨折的康复。Objective To analyze the effect of minimally invasive spinal surgery on osteoporotic vertebral fractures.Methods A total of 80 patients with osteoporotic vertebral fracture admitted to the Affiliated Hospital of Jiangnan University from January 2020 to December 2022 were selected as the study objects.According to odd-even grouping,they were divided into open group(n=35) and minimally invasive group(n=45).The open group received traditional open surgery,while the minimally invasive group received minimally invasive spinal surgery.The perioperative indexes,vertebral body status,healing and complications were compared between the two groups.Results After surgery,the time of operation and discharge in the minimally invasive group was shorter than that in the open group,and the differences were statistically significant(both P<0.05).The frontal height and middle height of injured vertebrae in the minimally invasive group were higher than those in the open group,and the Cobb Angle was lower than that in the open group,and the differences were statistically significant(all P<0.05).Japanese Orthopaedic Association scores and Activities of Daily Living scores in the minimally invasive group were higher than those in the open group,and Visual Analogue Scor scores were lower than those in the open group,and the differences were statistically significant(all P<0.05).The incidence of complications in minimally invasive group(0) was lower than that in open group(11.43%),and the difference was statistically significant(χ^(2)=5.414,P<0.05).Conclusion Minimally invasive spinal surgery has fewer complications,can quickly restore the height of the injured vertebra,reduce Cobb Angle,reduce vertebral pain,restore spinal function,and promote the rehabilitation of osteoporotic vertebral fracture.
