小儿深麻醉下气管拔管技术在小儿麻醉舒适化医疗中的应用

摘　　要：目的探讨小儿深麻醉下气管拔管技术在小儿麻醉舒适化医疗中的应用价值。方法选择2022年7月至2023年7月在河源市妇幼保健院行手术治疗的60例患儿作为研究对象,按照随机数字表法分为观察组(n=30)和对照组(n=30)。两组患儿手术均为气管插管全身麻醉下手术,术毕麻醉复苏过程中,观察组在深麻醉下拔管,对照组在清醒后拔管。比较两组心率(HR)、平均动脉压(MAP)、躁动情况、全麻苏醒期使用紧急气道处理例数及拔管后不良反应总发生率。结果观察组拔管时HR、MAP高于对照组,差异有统计学意义(P<0.05);观察组拔管时、清醒后5 min、返病房时镇静-躁动评分(SAS)低于对照组,差异有统计学意义(P<0.05);观察组紧急气道处理率、拔管后不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论在小儿麻醉中应用小儿深麻醉下气管拔管技术可减轻患儿术后躁动、降低不良反应发生,增加患儿围手术期舒适性和安全性,值得临床推广。Objective To explore the application value of tracheal extubation technology under deep anesthesia in pediatric anesthesia comfort medical treatment.Methods A total of 60 children who underwent surgical treatment in Heyuan Maternal and Child Health Hospital from July 2022 to July 2023 were selected as the study objects,and they were divided into observation group(n=30)and control group(n=30)according to random number table method.Both groups of children were operated under tracheal intubation and general anesthesia.During anesthesia resuscitation after operation,the observation group was extubed under deep anesthesia,while the control group was extubed after waking up.The heart rate(HR),mean arterial pressure(MAP),agitation,the number of emergency airway management and the total incidence of adverse reactions after extubation were compared between the two groups.Results The HR and MAP of the observation group were higher than those of the control group at extubation,and the differences were statistically significant(P<0.05).The sedation-agitation score(SAS)of the observation group was lower than that of the control group at extubation,5 min after waking up and returning to the ward,and the difference was statistically significant(P<0.05).The rate of emergency airway treatment and the total incidence of adverse reactions after extubation in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of trachea extubation under deep anesthesia in pediatric anesthesia can reduce postoperative agitation and adverse reactions,increase perioperative comfort and safety of children,and is worthy of clinical promotion.
