盐酸达克罗宁胶浆在小儿尿道下裂留置导尿中的应用

摘　　要：目的探讨盐酸达克罗宁胶浆用于小儿尿道下裂留置导尿的护理应用效果。方法选取2021年3月至2023年3月于赣州市妇幼保健院行尿道成形术治疗的尿道下裂患儿60例作为研究对象,采用随机数字表法将患儿分为A组(20例)、B组(20例)和C组(20例)。A组将无菌石蜡油涂抹于导尿管表面后留置导尿;B组将盐酸达克罗宁胶浆3~5 ml充分涂抹于导尿管表面后再留置导尿;C组将导尿前尿道洗净消毒后,将3 ml盐酸达克罗宁胶浆注入尿道,再用盐酸达克罗宁胶浆3~5 ml充分涂抹于导尿管后留置导尿,比较三组苏醒期Ricker镇静-躁动评分、返回病房后2 h膀胱刺激征的发生情况以及家属对患儿留置导尿的满意度。结果B组和C组Ricker镇静-躁动评分均低于A组,且C组低于B组,差异有统计学意义(P<0.05);比较三组膀胱刺激征发生率,差异有统计学意义(P<0.05);两两比较,A组膀胱刺激征发生率高于C组,差异有统计学意义(P<0.017);三组的满意度比较,差异无统计学意义(P>0.05)。结论盐酸达克罗宁胶浆用于小儿尿道下裂留置导尿,可有效减轻患儿苏醒期躁动,降低膀胱刺激征发生风险。Objective To explore the nursing effect of Dyclonine Hydrochloride Mucilage used for indwelling catheterization in children with hypospadias.Methods A total of 60 children with hypospadias who underwent urethroplasty in Ganzhou Maternal and Child Health Hospital from March 2021 to March 2023 were selected as the study objects.The children were divided into group A(20 cases),group B(20 cases)and group C(20 cases)by random number table method.In group A,aseptic paraffin oil was applied on the surface of the catheter and then catheterized with indwelling.In group B,Dyclonine Hydrochloride Mucilage 3-5 ml was fully applied on the surface of the catheter,and then indwelling the catheter.In group C,after the urethra was washed and disinfected before catheterization,3 ml of Dyclonine Hydrochloride Mucilage was injected into urethra,and then 3-5 ml of Dyclonine Hydrochloride Mucilage was fully applied to indwelling catheterization after catheterization.Ricker's sedation-agitation score during the recovery period,the occurrence of bladder irritation signs 2 h after returning to the ward,and the satisfaction of family members with indwelling catheterization were compared among the three groups.Results Ricker sedation-agitation score of group B and group C was lower than that of group A,and group C was lower than group B,the difference was statistically significant(P<0.05).There was statistical significance in the incidence of bladder irritation among the three groups(P<0.05).Pair-to-pair comparison showed that the incidence of bladder irritation in group A was higher than that in group C,with statistical significance(P<0.017).There was no significant difference in satisfaction among three groups(P>0.05).Conclusion Dyclonine Hydrochloride Mucilage used for indwelling catheterization in children with hypospadias can effectively reduce restlessness during recovery,reduce the risk of bladder irritation sign,and improve the satisfaction of family members with indwelling catheterization in children.
