彩色多普勒超声检查在糖尿病下肢血管病变中的诊断价值分析

摘　　要：目的分析彩色多普勒超声检查应用于糖尿病下肢血管病变诊断中的价值。方法选取2019年4月—2022年11月沂南县第三人民医院收治的40例糖尿病下肢血管病变患者,均经过数字减影血管造影(Digital Subtraction Angiography,DSA)确诊,记为糖尿病组,并选择同期40例健康者为对照,记为健康组。两组均接受彩色多普勒超声检查,对比两组血管内径,统计彩色多普勒超声检查检出结果、诊断效能。结果健康组股动脉、腘动脉、胫后动脉、足背动脉血管内径均大于糖尿病组,差异有统计学意义(P均<0.05);彩色多普勒超声检查真阳性38例,假阳性0例,假阴性2例,真阴性40例,敏感度95.00%,特异度100.00%,准确性97.50%,误诊率0,漏诊率5.00%,阳性预测值100.00%,阴性预测值95.23%。结论糖尿病下肢血管病变中应用彩色多普勒超声检查技术较为精准,诊断效能高,可以判断血管内径情况,将其用于糖尿病下肢血管病变诊断具有重要价值。Objective To analyze the value of color Doppler ultrasound in the diagnosis of diabetic lower extremity vascular disease.Methods 40 paients with diabetic lower extremity vascular disease admitted to the Yinan County Third People's Hospital from April 2019 to November 2022 were selected and were set as the diabetic group.The diag-nosis was confirmed by digital subtraction angiography(DSA).In the same period,40 healthy objects were selected as controls,who were set as the healthy group.All objects received color Doppler ultrasound.The vascular diameter were compared between the two groups.The detection result and diagnostic efficiency of color Doppler ultrasound were counted.Results The vascular diameter of femoral artery,popliteal artery,posterior tibial artery and dorsal foot artery in healthy group were higher than those in diabetic group,the differences were statistically significant(all P<0.05).There were 38 true positive cases,0 false positive cases,2 false negative cases and 40 true negative cases in the result of color Doppler ultrasound.The sensitivity was 95.00%,the specificity was 100.00%,the accuracy was 97.50%,the misdiagnosis rate was 0,the missed diagnosis rate was 5.00%,the positive predictive value was 100.00%and the negative predictive value was 95.23%.Conclusion The application of color Doppler ultrasound in the diagnosis of dia-betic lower extremity vascular disease is more accurate and has high diagnostic efficiency,which can judge the inter-nal diameter of blood vessels,and it has important value in the diagnosis of diabetic lower extremity vascular disease.
