摘　　要：目的:分析颅盆环牵引及后路内固定技术治疗重度脊柱侧弯的效果。方法:选取2021年1月—2023年10月北京优联医院收治的重度脊柱侧弯患者100例作为研究对象,采用随机数字表法分为对照组与观察组,各50例。对照组采用常规脊柱矫形手术方案治疗,观察组采用颅盆环牵引及后路内固定技术治疗。比较两组脊柱侧弯度、腰椎功能、疼痛程度、并发症发生情况。结果:治疗60d后,两组侧凸Cobb角及后凸Cobb角小于治疗前,且观察组小于对照组,差异有统计学意义(P<0.05)。治疗60 d后,两组Oswestry功能障碍指数、视觉模拟评分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P=0.046)。结论:颅盆环牵引及后路内固定技术治疗重度脊柱侧弯的效果显著,可降低脊柱侧弯度,增强腰椎功能,减轻疼痛,减少并发症发生风险。Objective:To analyze the effect of craniopelvic ring traction and posterior internal fixation in the treatment of severe scoliosis.Methods:A total of 100 patients with severe scoliosis were selected as research objects at Beijing Youlian Hospital from January 2021 to October 2023,and were divided into control group and observation group by random number table method,with 50 cases in each group.The control group was treated with conventional spinal orthopedic surgery,and the observation group was treated with craniopelvic ring traction and posterior internal fixation.Lateral curvature,lumbar function,pain and complications were compared between the two groups.Results:On day 60 after treatment,the scoliosis Cobb Angle and kyphotic Cobb Angle of the two groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).At 60 days after treatment,the Oswestry disability index and visual analog score of the two groups were lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.046).Conclusion:Craniopelvic ring traction and posterior internal fixation are effective in the treatment of severe scoliosis,which can reduce the curvature of the spine,enhance the function of the lumbar spine,relieve pain and reduce the risk of complications.
