增殖期手术联合曲安奈德术后球内注射治疗糖尿病视网膜病变的临床研究

摘　　要：目的:研究对增殖期糖尿病视网膜病变(Proliferating Diabetic Retinopathy,PDR)患者采用增殖期手术联合曲安奈德术后球内注射治疗方法的临床效果。方法:选取2021年1月至2022年12月期间我院收治的PDR患者82例共96眼作为研究对象。随机将患者分为对照组和研究组,对照组41例(47眼),研究组41例(49眼)。对照组给与增殖期手术治疗;研究组在对照组基础上联合曲安奈德术后球内注射治疗。分析对比两组的临床疗效、临床相关指标、消退情况及并发症。结果:研究组治疗总有效率为明显高于对照组(P<0.05)。治疗后,两组的眼压、黄斑中心凹厚度和最佳矫正视力相比术前均显著降低,且研究组的降低幅度显著高于对照组(P<0.05)。研究组新生血管消退率和无灌注区消退率均显著高于对照组(P<0.05)。研究组术后并发症发生率显著低于对照组(P<0.05)。结论:对PDR患者实施手术联合曲安奈德术后球内注射治疗有效提高治疗效果,加速无灌注区和新生血管消退,有效改善视功能,降低并发症。Objective:To investigate the clinical effect of Proliferating Diabetic Retinopathy(PDR)combined with triamcinolone acetonide injection after proliferating diabetic retinopathy(PDR).Methods:A total of 82 PDR patients with 96 eyes admitted to our hospital from January 2021 to December 2022 were selected as the study objects.The patients were randomly divided into control group(41 cases(47 eyes))and study group(41 cases(49 eyes)).The control group was given proliferative surgery.On the basis of the control group,the study group was treated with triamcinolone acetonide injection after operation.The clinical efficacy,clinical indicators,regression and complications of the two groups were analyzed and compared.Results:The total effective rate of the study group was significantly higher than that of the control group(P<0.05).After treatment,the IOP,macular fovea thickness and optimal corrected visual acuity in both groups were significantly reduced,and the reduction in the study group was significantly higher than that in the control group(P<0.05).The regression rate of neovascularization and non-perfusion area in the study group were significantly higher than those in the control group(P<0.05).The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion:Operation combined with intrabubular injection for PDR patients can effectively improve the therapeutic effect,accelerate the regression of non-perfusion area and neovascularization,effectively improve visual function and reduce complications.
